瘢痕疙瘩不同部位微淋巴管密度的研究

摘    要:目的探讨瘢痕疙瘩微淋巴管密度与其呈侵袭性生长的相关性。方法手术切取瘢痕疙瘩10例,根据瘢痕疙瘩的特点将每例瘢痕疙瘩分为浸润部位、增生部位及边缘正常皮肤,非瘢痕疙瘩者的瘢痕疙瘩好发区域的正常皮肤作为对照,应用免疫组织化学染色,对瘢痕疙瘩增生部位、浸润部位、边缘正常皮肤及非瘢痕疙瘩者的正常皮肤微淋巴管密度进行检测分析。结果瘢痕疙瘩边缘正常皮肤微淋巴管密度(12.563±1.803)条/ram。,与非瘢痕疙瘩的正常皮肤微淋巴管密度(11.071土2.645)条/mm。差异无统计学意义(t=1.389,P〉O.05);瘢痕疙瘩浸润部位微淋巴管密度(27.315士7.430)条/mm。,明显高于增生部(17.375±6.221)条/mm。及边缘正常皮肤(12.563±1.803)条/m㎡,差异有统计学意义(X^2=16.8,P〈O.05);瘢痕疙瘩增生部与边缘正常皮肤微淋巴管密度的表达差异有统计学意义(t=2.276,P〈O.05)。结论瘢痕疙瘩浸润部的微淋巴管密度增高可能与瘢痕疙瘩坚侵袭性生长有一定关系。

Abstract:Objective To study the lymphatic microvessel density of keloid and correlation of invasive growth. Methods Invasive part, proliferative part and peripheral normal skin of keloid pa- tients (n: 10) were collected by surgical excision, and normal skin of keloid and non-keloid patients as control. I.ymphatic vessel density of different parts of keloid and normal skin were investigated by im munohistochemical staining using the lymphatic endothelium specific marker D2-40. Results There were no significant differences between lymphatic microvessel density of peripheral normal skin of ke loid (12. 563 ± 1. 803) n/mmz and normal skin of non-keloid patients (11. 071±2. 645) n/m㎡ (t=1.389, P〉0.05); Lymphatic mierovessel density of invasive part of keloid (27. 315 ± 7.430) n/ram2 was significantly higher than those of proliferative part of keloid (17. 37.5 6. 221) n/m㎡ and normal skin (12. 563±1. 803) n/mm2 (X^2=16.8, P〈0.01); there were signifi cant differences between proliferative parts of keloid and normal skin (t: 2. 276, P〈0.05). Conclu sions Lymphatic microvessel density increase in the invasive part of keloid may have a certain rela tionship with invasive growth.
