2型糖尿病患者铁代谢的相关性研究

摘    要:目的对2型糖尿病患者体内铁代谢状况进行分析。方法选取2009年6月至2012年1月攀枝花市第二人民医院收治的2型糖尿病患者90例作为糖尿病组,另选取80例健康体检者作为健康体检组,检测并比较两组受试者血清铁、转铁蛋白(TRF)、铁蛋白(SF)、运铁蛋白饱和度(TS)、空腹血糖(FPG)、糖化血红蛋白(HbA1c)水平及胰岛素抵抗指数(HOMA-IR)等代谢指标。结果糖尿病组患者FPG、HOMA-IR水平、HbA1c、血清铁、SF、TRF、TS均显著高于健康体检组(t=8.64,8.57,7.98,6.75,3.75,15.68,6.75,P<0.05);经Pearson检验,血清铁与SF含量呈正相关(r=0.384,P<0.05),血清铁、SF与TRF均呈负相关(r=-0.472、-0.685,P<0.05),血清铁、SF、TRF与CysC呈正相关(r=O.674,0.637,0.602,P<0.05);HOMA-IR与SF呈正相关(r=0.653,P<0.05)。结论糖尿病患者体内存在以血清铁、SF、TRF增高为主的铁超负荷,其可作为糖尿病早期监测指标。

Correlation Study of Iron Metabolism and Type 2 Diabetes

Abstract:Objective To analyze the iron metabolism in patients with type 2 diabetes. Methods A total of 90 patients with type 2 diabetes at Panzhihua Second People's Hospital from Jun. 2009 to Jan. 2012 were selected( diabetic group) ,and 80 healthy persons as control group. Metabolic indicators such as serum Fe,TRF,SF,TS,FPG,HbAIC and HOMA-IR were tested and compared. Results FPG,HOMA-IR,HbA1C, serum Fe,SF, TRF and TS of diabetic group were significant higher than control group ( t = 8.64,8.57, 7.98,6.75,3.75,15.68,6.75, P 〈 0.05 ). By Pearson correlation analysis, serum Fe was positively correla- ted with SF( r = 0. 384, P 〈 0.05 ), serum Fe and SF were negative correlated with TRF( r = - 0. 472, - 0. 685, P 〈 0.05 ), serum Fe, SF and TRF were positively correlated with CysC ( r = 0. 674,0. 637,0. 602, P 〈0.05) ,HOMA-IR was positively correlated with SF(r =0.653,P 〈0.05). Conclusion Iron overload such as serum Fe, SF and TRF exist in patients with type 2 diabetes. Serum Fe, SF and TRF can be used as early indicators for monitoring diabetes.
