中西医结合治疗小儿过敏性紫癜临床疗效分析及安全性评价

目的：探讨自拟中药汤剂联合西药治疗小儿过敏性紫癜的临床疗效及其安全性,为临床诊断治疗提供参考。方法：将本组83例小儿过敏性紫癜患者根据随机数字表法患儿随机分为治疗组（n=43）和对照组（n=40）。对照组给予常规西药治疗,治疗组在对照组治疗基础上结合自拟中药汤剂治疗,对比分析两组治疗后临床疗效、临床症状消失时间、不良反应发生情况及随访1年后复发情况,两组患儿均以10 d为1个疗程。结果：治疗组治疗后总有效率（93.02%）显著高于对照组（77.50%）,且有显著性差异（P〈0.05）;治疗组治疗后患儿临床症状消失时间（包括腹痛、紫癜及关节疼痛）均明显短于对照组,且具有显著性差异（P〈0.05）;治疗组治疗后不良反应发生率（6.98%）低于对照组（12.50%）,但无显著性差异（P〉0.05）;治疗后随访1年治疗组复发率（16.28%）显著低于对照组（40.00%）,且有显著性差异（P〈0.05）。结论：自拟中药汤剂联合西药治疗小儿过敏性紫癜患儿取得了明显的临床疗效,可明显提高临床症状消失时间,不良反应较轻且复发率低,临床应用安全可靠,具有重要意义。

Clinical Efficacy of Integrative Medicine in Children with Allergic Purpura and Its Safety Evaluation

Objective：To investigate the clinical efficacy and safety self- made decoction combined with Western medi-cine in children allergic purpura and to provide reference for the clinical diagnosis and treatment. Methods： 83 cases of pediatric patients with allergic purpura were randomly divided into treatment group（n = 43） and control group（n = 40）according to a random number table in children. The control group received conventional western medicine and treatment group used combined therapy. The clinical efficacy of comparative analysis of the two groups after the treatment,the clinical symptoms disappeared time,adverse events,and follow- up of recurrence after one year were compared. 10 d was a course. Results： The total effective rate（93. 02%） in treatment group was significantly higher than that of control group（77.50%） after treatment and there was significant difference（P〈0. 05）. After treatment,clinical symptoms disappeared time（ including abdominal pain,purpura and joint swelling and pain） was significantly shorter than that of the control group with a significant difference（ P〈0. 05）. After treatment,the incidence of adverse reactions（6. 98%） in treatment group was lower than that of control group（12. 50%） with no significant difference（P〈0. 05）. After 1- year follow- up after treatment,the relapse rate in treatment group（16. 28%） was significantly lower than that of the control group（40. 00%） and there was a significant difference（ P〈0. 05）. Conclusion： Self- made decoction combined with Western medicine in children with allergic purpura has achieved significant clinical efficacy. It can significantly improve the clinical symptoms of time and has lighter adverse reactions and low recurrence rate. The clinical application is safe and reliable and has great significance.
