高频彩色多普勒超声多因素分析预测颈部浅表淋巴结病变性质

目的评价各种预测肿大淋巴结病变性质的超声影像学指标，并建立一种可行的预测模型。方法对93例病人的107个颈部浅表淋巴结进行超声检测，指标包括L／S、髓部回声、血管类型、RI、PI、Vmax、Vmin，用Logistic回归分析进行变量筛选、组合及建立预测模型。结果根据Logistic回归分析的参数评估结果，评分公式为：5—20L／S 20(髓部回声特征) 2(VS评分)-3RI 7PI。将评分值大于0作为恶性诊断标准，其敏感性84．0％，特异性75．4％。结论一系列独立变量的联合应用优于单一诊断指标，它可以提供诊断恶性淋巴结不易混淆可靠的概率参考.

A Multivariate Probability Prediction of Superficial Lymphadenopathy by High Frequency Color Doppler Ultrasonography

Objective To assess various sonographic parameters in predicting malignant superficial lymphadenopathy in order to set up a reliable prediction. Methods One hundred and seven cervical lymph node from 93 consecutive patients were used for building the prediction model. Sonographic variables, including US, medulla echogenicity, VS, RI, PI, Vmax, Vmin, were analyzed with multivariate logistic regression to evaluate the joint effect of a set of independent variables and establish a prediction rule for malignant lymphadenopathy by statistic method. Results Scoring scale was designed as 5-20(L/S)+20 (medulla echo)+2(VS)-3(RI)+ 7 (PI) according to the parameter estimates of multivariate logistic regression analysis. Cut-off value of score>0 as malignancy resulted in 84.0% sensitivity and 75.4% specificity. Conclusion The joint effect of a set of independent variables is better than any single parameter, this prediction rule could provide the physician a clear and reliable probability reference for managing lymphadenopathy.
