主动脉夹层累及颈动脉的超声图像特点

摘    要:目的评价主动脉夹层累及颈动脉的超声图像特点。方法采用PILIPS7500彩色多普勒超声诊断仪,对29例患者经常规心脏超声检查诊断为主动脉夹层后,切换至颈动脉检查模式,观察颈动脉内膜是否累及,夹层撕裂至颈动脉的图像特点、血流状态。结果 29例患者中,夹层累及颈动脉7例,占24.1%,均发生在Standford A型患者。其中4例颈动脉超声清晰显示撕裂飘浮的内膜,真假腔血流状态不同。左颈总动脉或右颈总动脉假腔内血栓形成、真腔狭窄各1例。另1例右颈总动脉近完全闭塞,无血流信号。结论颈动脉超声能准确评估主动脉夹层患者颈动脉受累情况。

Abstract:Objective To evaluate the ultrasound characters of carotid artery in aortic dissection with carotid artery involved.Methods Twenty-nine patients were diagnosed aortic dissection by echocardiography using a available PILIPS 7500 ultrasound system.Examination of both common carotid arteries was performed by shifting to the mode of high frequency transducer to evaluate whether common carotid arteries were involved and evaluate the signs of common carotid artery dissection,and the state of blood flow in carotid artery.Results Ninteen patients were diagnosed standford A aortic dissection among 29 patients.Carotid arteries were involved in 7 of them(24.1%).Four patients with perfect image were characterized by an intimal flap separating the true and false lumen in 2-dimensional mode,and the different blood flow state in color doppler flow imaging mode.Thrombi were found in false lumen and caused true lumen stenosis in left or right common carotids artery among 2 patients.Occlusion of right common carotid artery and almost no moving blood flow was found in another patient.Conclusion Carotid artery ultrasound can be used accurately to evaluate aortic dissection with carotid artery involved.
