妊娠期糖尿病27例临床分析

目的了解妊娠期糖尿病（GDM）的发病率及其母儿并发症。方法采用前瞻性的研究方法,对2005年1月至2006年12月在苏州大学附属第一医院建卡的976例孕妇通过50g葡萄糖负荷试验（GCT）进行糖筛查,阳性者再做75g糖耐量试验（OGTT）。27例GDM阳性为糖尿病组,以糖筛查正常的100例孕妇作为对照组,对其临床资料进行分析。结果筛查976例孕妇中GCT阳性者84例（8.61%）,总糖耐量异常者37例（3.79%）,其中GDM27例（2.77%）,糖耐量异常（GIGT）者10例（1.02%）。GDM组和对照组相比,巨大儿和羊水过多的发生率有明显差异（P〈0.05）,而子痫前期、胎儿窘迫、早产等并发症,两组相比差异无统计学意义（P〉0.05）。GDM组剖宫产率为59.26%,对照组为37%。结论进行糖尿病筛查及时发现妊娠期糖尿病,并对GDM患者进行严格的管理,有利于母婴健康。

Clinical Analysis of 27 Gestational Diabetes Mellitus Cases

Objective To explore the incidence and complications of gestational diabetes mellitus（GDM）.Methods Screening for GDM was performed in 976 pregnant women during Jan.2005 and Dec.2006 through 50g glucose challenge test（GCT）,if positive,a subsequent diagnostic 75g oral glucose tolerance test（OGTT） was performed.The GDM group included 27 cases which were diagnosed as GDM,and the control group included 100 cases of normal pregnant women chosen randomly.Clinical data was compared between GDM group and control group.Results Out of all cases,there were 84 cases with positive GDM screening results（8.61%）,and 27 cases with GDM（2.77%）.There was significant difference in the incidence of macrosomia and polyhydramnios between the GDM group and the control group（P 〈0.05）,whereas there was no significant difference in incidence of preeclampsia,fetal distress and preterm labor in the two groups（P 〉0.05）.Conclusion Screening for and management of GDM patients is of great benefit to maternal and fetal health.
