P53和Bcl-2在胰腺癌中的表达及其临床意义

目的通过检测P53、Bc1-2蛋白在胰腺癌及正常胰腺组织中的表达,探讨其表达的临床意义。方法应用免疫组化法检测组织芯片中P53、Bc1-2蛋白在88例胰腺导管腺癌和11例正常胰腺组织的表达情况。应用人工读片和激光扫描细胞仪两种方法检测分析其相关性及临床意义。结果 P53和Bcl-2蛋白在正常胰腺组织的阳性率分别为0和100%（11/11）,胰腺癌组织中的阳性率分别为63.6%（56/88）和54.5%（48/88）,两种蛋白在胰腺癌组织和正常胰腺组织中的阳性率差异均具有统计学意义（均P〈0.05）;两者表达与原发性胰腺导管腺癌临床分期、病理分级及有无淋巴结转移有关（P〈0.05）。经Spearman秩相关检验,P53蛋白和Bc1-2蛋白相关系数为-0.2254,成负相关表达（P〈0.05）。Cox风险分析提示,P53蛋白、Bc1-2蛋白阳性表达患者生存时间明显缩短（P〈0.05）,生存单因素分析胰腺癌的病理分级、临床分期及有无淋巴结转移、P53蛋白和Bc1-2蛋白表达有关（P〈0.05）。结论 P53、Bc1-2蛋白在胰腺癌的发生发展中可能起着重要的作用,P53、Bc1-2蛋白表达的联合检测有助于胰腺癌的预后判断。

Expressions of P53 and Bcl-2 in Pancreatic Carcinoma and Its Clinical Significance

Objective To study the expression and clinical significance of P53,Bcl-2 in pancreatic carcinoma.Methods The expression of the three proteins was detected by SP immunohistochemical method in 88 cases with pancreatic carcinoma and 11 cases of nontumorous pancreatic tissue,Their relationships with clinic-pathological features and survival rate was also analyzed by factitial and LSC.Results The positive ratio of P53 protein expression in nontumorous pancreatic tissue and pancreatic carcinoma was 0 and 63.6%,The positive ratio of Bcl-2 protein expression in non tumorous pancreatic tissue and pancreatic carcinoma was 100% and 54.5%,respectively,The difference was significant（P0.05）.The expression level of P53 protein,Bcl-2 protein was correlated with clinical stages,pathology graduation and metastasis of lymph nodes（P0.05）.The P53 protein and the Bcl-2 protein correlation coefficient was-0.225 4 by the Spearman rank test,both were inverse correlation.The Cox risk analysis revealed that the patient life with positive expression of p53 protein and the bc1-2 protein reduced obviously.The single-factor analysis revealed that the clinical stages,pathology graduation,metastasis of lymph nodes expression and the expression of p53 protein and bc1-2 protein was significantly correlated with the prognosis of pancreatic carcinoma.Conclusion The p53 protein and bc1-2 protein plays an important role in growth,invasion and metastasis of pancreatic carcinoma.Their expression’s union examination is helpful for the prognosis judgment of cancer.
