摘    要:目的:探讨螺旋CT多平面重组(MPVR)和表面遮盖显示(SSD)在胫骨平台骨折中的应用价值。方法: 32例胫骨平台骨折经X线平片、CT扫描后MPVR和SSD后处理,对骨折进行分型,评估骨折程度,选择合适的治 疗方案。结果:32例经MPVR重建后均能清晰地显示各种骨折,可以直接测量骨折劈裂和塌陷的大小。SSD能准 确评估骨折劈裂的方向和塌陷的面积。27例行手术治疗,手术入路根据SSD来确定,手术结果证实了SSD和 MPVR显示的骨折情况。结论:螺旋CT的MPVR和SSD重建有助于胫骨平台骨折碎片的立体定位和正确分型, 对手术方案的制定有重要意义。

Abstract:Objective: To study the value of multiplanar volume reconstruction (MPVR) and surface shaded display (SSD) with helical CT in tibial plateau fracture. Methods: Tibial plateau fractures in 32 patients were imaged by plain radiography an axial CT scans. The source images of CT were reconstructed by MPVR and SSD techniques. Based on the imaging findings, the fractures were classified and appropriately treated. Results: All the fractures of 32 cases with MPVR showed well. The splitting and degree of depression could be directly measured on coronal plane reconstruction with MPVR. SSD was useful for the evaluation of the direction of splitting and extent of the depression, and the selection of surgical access. Twenty-seven cases underwent operation, and their surgical findings proved the demonstration of fractures on SSD and MPVR. Conclusion: MPVR and SSD with helical CT were very helpful for stereoscopic visualization of fragments and accurate classification of tibial plateau fractures, and played an important role in the selection of surgical approaches and the evaluation of prognosis.
