玻璃体切割术后早期高眼压的相关因素分析

Objective To investigate the incidence and risk factors of IOP elevation in patients who undergone vitreoretinal surgery. Methods Retrospectively studied 119 patients(132 eyes)who underwent vitreoretinal surgery in our department and analyzed the factors that might be related to the state of high ocular pressure. The ocular pressures in both preoperative and postoperative periods were detected by Goldmann tonometry examination, and the ocular hypertension was defined as IOP more 24mmHg. Results The high IOP occurred in 50 eyes after operation(37.88%). The rate of IOP elevation with the different sex and different age range was statistically significant(P <0.05). The patients who had intraocular surgery before were more likely to have IOP elevation than the ones without intraocular surgery(P<0.05). The rate of IOP elevation with scatter endophotocoagulation or partial endophotocoagulation was 46.77%, 30% respectively, the difference was statistically significant(x2=3.932, γ=1> P=0.047). The rate of IOP elevation with intraocular tamponade of silicone oil, C2F6, C3F8or none was 48%, 41.18%, 57.14%, 11.76% respectively, the difference was statistically significant(x2=14.395, γ =3, P=0.002). Intraocular surgery, intraocular tamponade were the risk factor by Logistic regression analysis. Conclusions IOP elevation is a common complication of vitrectomy. The previous intraocular surgeries, certain primary eye disease and combined ocular procedures are the risk factors of IOP elevation after PPV.
