78例早发型重度子痫前期的临床分析

目的探讨早发型重度予痫前期对围产期母婴预后的影响。方法对78例早发型重度子痫前期患者进行回顾性分析。根据其孕周不同分为3组，即A组（孕周〈28周）、B组（28周≤孕周〈32孕周）、C组32孕周≤孕周〈34孕周）；根据治疗时间分为4组即Ⅰ组（治疗时间≤48h）、Ⅱ组（治疗时间48h～7d）、Ⅲ组（治疗时间7～20d）、Ⅴ组（治疗时间〉20d）。结果早发型重度子痫前期发病孕周越早，患者的并发症的发生率越高，A、B、C各组间差异无统计学意义（P〉0．05）；新生儿病死率和围产儿病死率随着发病孕周的延长及治疗时间的延长而显著降低，差异有统计学意义（P〈0．01）。结论早发型重度予痫前期严重影响母婴预后，适当期限内的保守治疗有助于改善母儿结局。

Clinical Analysis on 78 Cases of Early Onset Severe Preeclampsia

Objective To study the impact of early onset severe preeclampsia on the prognosis of perinatal mothers and infants. Methods A retrospective study was carried out on 78 patients with early offset severe preeclampsia. They were divided into three groups according to gestational weeks： group A（ 〈 28 weeks）, group B （ ≤28 -32 weeks） and group C （ ≤32 -34 weeks）. And four groups was defined according to duration of expectant management ： group （ ≤ 48 h）, group （ 〈 48 h 〈 7 d）, group （7 ≤ 20 d） and group（ 〉20 d）. Results The incidence of complications increased with the early attacks of early onset severe preeclampsia （less gestational weeks） ;No significant difference among the three groups was found （P 〉 0.05 ）. The neonatal mortality and perinatal mortality rate decreased with the prolongation of onset week and time of treatment （ P 〈 0.01 ）. Conclusion The early onset severe preeclampsia badly influences the prognosis of perinatal mothers and infants. The expectant treatment can improve the outcomes of perinatal mothers and infants.
