纵隔巨大淋巴结增生症的诊断和外科治疗

目的为了提高对纵隔巨大淋巴结增生症的认识，探讨其诊断方法和外科治疗特点。方法从1989年3月～2004年6月通过6例经我科手术诊治的纵隔巨大淋巴结增生症的病例分析，结合文献复习，对本病的临床特点，影像学表现，诊断及手术治疗进行讨论。结果6例病理均为透明血管型巨大淋巴结增生症。术后病理诊断与术前诊断相符率为33．3％（2／6），随访5月-8年，均无复发。结论纵隔巨大淋巴结增生症为罕见病。术前诊断率低，诊断依靠病理组织学确定，本病首选治疗为手术切除，预后良好。

Diagnosis and surgical treatment of mediastinal giant lymph node hyperplasia

Objective To improve the recognition of mediastinal giant lymph node hyperplasia(GLNH) and investigate it's diagnostic method and feature of surgical treatment. Methods From March 1989 to November 2004, 6 cases with mediastinal GLNH received surgical treatment were analyzed retrospectively and combined with literatures were reviewed. It was discussed in the clinical feature, imaging manifestation,diagnosis and operative treatment. Results The pathologic diagnosis was transparent vascular type GLNH in all cases. Coincidental rate between postoperative pathologic diagnosis and preoperative diagnosis were 33.3%(2/6). Six cases were followed up from 5 months to 8 years, and no recurrence was found. Conclusion Mediastinal GLNH is a rare disease. It's preoperative diagnostic rate is very low. According to the pathohistologic biopsy, the diagnosis can be confirmed. Operation is the first choice for GLNH and it's prognosis is very well.
