婴幼儿缺铁性贫血治疗前后脑干听觉诱发电位的变化

摘    要:对50例缺铁性贫血(IDA)患儿补铁前及其中45例补铁后脑干听觉诱发电位(BAEP)的结果进行分析,结果提示缺铁性贫血可引起小儿听觉传导通路的周围性损害,主要表现为BAEP波形分化不良,反应阈提高,Ⅰ,Ⅲ,Ⅴ波绝对潜伏期(PL)延长,少数缺铁性贫血患儿Ⅰ-Ⅲ峰间期(IPL)达临界值,而Ⅲ-Ⅴ波IPL无异常改变。经补铁治疗,贫血纠正后,BAEP各参数恢复正常,说明这种听觉功能改变是可逆的。提示:对婴幼儿缺铁性贫血应早期发现,早期治疗。

Abstract:Auditory brainstem evoked potentials test was carried out in 50 children with iron deficiency anemia and reexamined in 45 children after iron supplementation.The results suggested that auditory function can be affected by iron deficiency anemia which led mainly to impairments of auditory peripheral pathways and likely not to brainstem.The impaired position was probably in the cochlea hair cell.The hearing threshhold was elevated and poor wave patterns were observed.The latency of wave Ⅰ,Ⅲ,Ⅴ was extended,the interpeak interval of wave Ⅰ-Ⅲ of some children was expanded to marginal limit,but that of wave Ⅲ-Ⅴ was not changed.The blood indicators recovered to normal levels after 3 months iron supplementation,and the BAEP parameters also recovered.It was concluded that the impairments of auditory pathways caused by iron deficiency anemia was reversible.Early diagnosis and treatment of anemia may be important in children.
