肥胖症儿童血浆脂联素与胰岛素水平的相关性

目的探讨血浆脂联素和胰岛素水平与儿童肥胖症发病的关系。方法选择2004～2007年于我院初诊并住院诊治的肥胖儿童40例和健康儿童50例,设为肥胖组及对照组。测定两组的空腹血清脂联素、胆固醇、三酰甘油、低密度脂蛋白-胆固醇、高密度脂蛋白-胆固醇、血糖和胰岛素,并分析脂联素与体重指数（BMI）、血脂、血糖和胰岛素的相关性。结果①肥胖组血浆脂联素低于对照组（P〈0.01）;肥胖组的血糖和胰岛素水平显著地高于对照组（P〈0.01）;②肥胖儿童的血脂各指标与对照组相比,LDL-C和TG明显高于对照组（P〈0.05、〈0.01）;③相关性分析：血浆脂联素与BMI,TG,空腹血糖,空腹胰岛素呈负相关（P〈0.01）。结论脂联素水平的变化与儿童肥胖症的发病有关,是影响空腹血糖、空腹胰岛素水平的重要因素。

The serum adiponectin levels and insulin in obese children

Objective To investigate the association of plasma adiponectin and insulin levels with childhood obesity. Methods From 2004 to 2007,40 obese children and 40 normal controls were enrolled in the study. The ELISA technique was used to determine the levels of plasma adiponectin. The serum levels of triglyceride （TG）, total chalesterol （TCH）, low-density lipoprotein-cholesteroul （ LDL-C）, high-density lipoprotein-cholestel ＇oi （ HDL-C ） were assayed with turbidimetry. Serum glucose Was measured by automatic biochemistry analyzer and serum instdin was detemined by eleetrocbemilumiflescence immunoassay. The correlation of plasma adiponectin with body mass index（ BMI）, blood lipids, serum glucose end insulin were analyzed. Results ①The levels ofplasma adiponectin decreased in obese children, as compared with the control group（ P〈0. 01 ）. Andthe serum insulin and glucose levels in obese children were significantly higher than those in control（P〈0. 01 ）. ②LDL-C and TG was found to have significant differences between obese and normal control children（P〈0.01 ）.③Plasma adiponectin level showed negative correlation with BMI,TG,fasfing glucose and fasting insulin（P〈0. 01 ）. Conclusion Based on the biologic effects ofadiponectin and the changes in obese children. It is possible that alteration in plasma adiponectin is correlated with child obesity. Serum adiponectin levels are negatively corelated with fasting glucose ,fasting insulin.
