鼻咽癌与多发癌160例临床分析

本文报告１９６３年３月－１９９１年８月我院诊治的鼻咽癌合并多发癌１６０例，其中鼻咽癌放疗后并发其他原发癌瘤者１１４例（第一组）；与鼻咽癌同时发生或其他原发癌治疗后发生鼻咽癌者４６例（第二组）。第一组患鼻咽癌时作根治性放疗；第二组发生另一原发癌前从未接受过放疗。结果为第一组另一原发癌发生于头颈部者８０例，占６９．０％。其它部位者３６例，占３１．０％；第二组则分别为３６．２％和６３．８％，差别显著。我

cases of nasopharyngeal cancer(NPC)associated with multipleprimary tumors were reported,which were treated from 1963 to 1991 in our hospital.All the cases were divided into two groups: the firSt,in 114 cases curative radiotherapywas given for nasopharyngeal cancers and the new malignancies subsequently developed.The second in 46 cases, nasopharyngeal cancers developed simultaneously with otherprimary tumors or NPC occurred after other tumor,and they had never received anyradiation before the other primary cancers. 80 ceses of the first group(68 % )were foundto have malignancies(66 cases of carcinomas and 14 cases of sarcomas)in the head andneck regions,36 cases(31%),in the other parts of the body. However,in the secondgroup,the respective proportions were 36.2 % and 63.8%,the differehce was marked,therefore,we believe that radiation may induce malignancies.The latent periods rangedfrom 1 to 26 years. In both groups,there were some patients with fa milial history of cancer,and thissuggested that nasopharyngeal cancer associated with multiple primary tumors may berelated to genetically determined susceptibility to carcinogenesis. Finally,the tleatment of secondacy mutiple primary tumors is the same as the ori-ginal cancers.We should try to give them curatiye treatment, even to the advanced si-tuations,palliative treatment should be given and a better result usually could be ob-tained.The cases in this group died mostly from uncontrolled diseases or metastases.
