铜绿假单胞菌的耐药性监测

摘    要:目的 分析2007至2009年我院345株铜绿假单胞菌(PA)对抗菌药物的耐药情况.方法 细菌鉴定和药敏试验采用VITEK 2 Compact全自动细菌培养鉴定仪,统计方法 采用Whonet 5.3软件.结果 PA的标本来源主要为痰液,占分离总数的83.8%(289/345);其次为尿液(20株)和分泌物(20株);病区分布前3位为呼吸内科(102株)、神经内科(65株)和脑外科(36株).耐亚胺培南铜绿假单胞菌(IRPA)分离率为8.7%(28株);PA和IRPA对四环素、复方新诺明、氨苄西林、氨苄西林/舒巴坦、第一代和第二代头孢菌素耐药性均高(>91%),对哌拉西林/他唑巴坦和妥布霉素耐药性均低(<33%);PA对头孢他啶、头孢吡肟、左氧氟沙星、哌拉西林、庆大霉素耐药性较低(15%~38%),但IRPA对上述药物的耐药性较高(67%~86%),二者差异有统计学意义(P<0.05).结论 PA是院内感染的主要病原菌,IRPA对大多数抗菌药物耐药,应及时监测耐药株,采取有效的感染控制措施.
Abstract:
Objective To analyze the drug resistance of antibacterials of 345 pseudomonas aeruginosa(PA) and Imipenem resistant pseudomonas aeruginosa (IRPA) from 2007 to 2009. Methods The bacteria identification and drug sensitive test adopted the VITEK 2 Compact Full automaticity bacterial culture identification apparatus. Results The sources of PA sample were mainly from sputamentum, urine and secretion. The top three infected regions were respiration medicine, neurology and brain surgery. The dissolution rate of IRPA was 8.7% . The drug resistances of PA and IRPA to tetracycline, sinomin compositea, ampicillin, ampicillin/sulbactam, first and second cephalosporin were averagely higher than 91%. The drug resistances of PA and IRPA to piperacillin-tazobactam sodium and tobramycin were lower than 33%, which did not show significant change (P>0.05). The drug resistance of PA to ceftazidime, cefepime, levofloxacin, piperacillin and gentamicin (from 15% to 38%) was lower than IRPA (from 67% to 86%) (P<0.05). Conclusion PA is the infected main pathogenicbacteria in hospital infection.
