高分辨率CT对眼眶内侧壁骨折的诊断

摘    要:目的分析和评价高分辨率CT对眼眶内侧壁骨折的诊断价值。方法回顾性分析38例(45眼)眼眶内侧壁骨折的HRCT资料。男31例(37眼),女7例(8眼),均行HRCT检查,层厚2mm,眼眶靶扫描,骨算法重建成像。结果38例(45眼)眶内壁骨折中,眼眶爆裂性骨折27例(27眼),直接骨折7例(13眼)及复合骨折4例(5眼)。直接征象为眶内壁骨质连续性断裂、断端分离、移位22例(25眼),骨质粉碎6例(8眼),眶壁曲度异常或塌陷10例(12眼)。结论高分辨率CT能细致地显示眼眶内侧壁的解剖结构,能准确地显示眼眶内侧壁骨折的部位、程度和并发症,具有重要的临床价值。

Abstract:Objective To evaluate HRCT in the diagnosis of the fracture of medial orbital wallMethods HRCT findings in 45 eyes of 38 patients(31 males and 7 females) with fracture of medial orbital wall were retrospectively analyzed.HRCT was performed with slice thickness of 2mm,and bone algorithm.Results In 45 eyes of 38 cases of the fracture of medial orbital wall,13 eyes of 7 cases were direct fracture,27 eyes of 27 cases were burst fracture and others were complex fracture.The HRCT findings of the fracture of medial orbital wall were followed:orbital bone discontinuity and displacement(25 eyes of 22 cases),comminuted bone(8 eyes of 6 cases),orbital flexion abnormality or excavation(12 eyes of 10 cases).Conclusions HRCT can well demonstrate the anatomy of medial orbital wall,and accurately display the bony details including location,extent and secondary changes of the fracture of medial orbital wall,have important clinical value for diagnosis of the fracture of medial orbital wall.
