米非司酮联合利凡诺用于中期妊娠引产中胎盘胎膜剥离的临床观察

摘    要:目的探讨米非司酮联合利凡诺用于中期妊娠引产术中胎盘胎膜剥离的情况,减少产后出血、宫内感染等并发症的发生。方法400例孕妇中选出16~27^+6周身体健康、自愿要求终止妊娠患者300例,随机分为研究组150例和对照组150例。研究组一次口服米非司酮150mg,同时行羊膜腔内注射利凡诺100mg;对照组按常规行羊膜腔内注射利凡诺100mg引产。结果研究组与对照组胎盘胎膜剥离完整程度、清宫率、产后2h出血量及产后2周内感染率相比差异有显著性。结论米非司酮联合利凡诺用于中期妊娠引产,使蜕膜剥离完整,利于胎儿及附属物的顺利排出,减少了产后出血量和胎盘胎膜残留,降低了清宫率,降低了感染率,减轻了患者痛苦,是一种较理想、切实可行的方法,值得推广。

Abstract:Objective To explore the effectiveness of mifepristone in combination with rivanol for stripping placenta and fetal membrane and preventing complications of postpartum hemorrhage and intrauterine infection in termination of mid - trimester pregnancy. Methods Totally 300 healthy women who asked for termination of 16-- 27 +6 weeks gestation were divided randomly into the control group and the observation group. The former was treated only with 100 mg rivanol by intra- amniotic injection, besides that, the later was added 1.50 mg mifepristone orally. Results There were significant differences in stripping placenta and fetal membrane, the rate of suction curettage, postpartum hemorrhage, and puerperal infection between the two groups. Conclusions Mifepristone combined amniocentesis injection of rivanol is an effective, safe, simple, and quick method in termination of mid- trimester pregnancy.
