缺血性脑血管病和冠心病患者Aid与ET-1相关性研究

摘    要:目的:探讨缺血性脑血管(ICVD)和心血管(CHD)疾病患者醛固酮(aldosterone,Ald)与内皮素(endo-thelin-1,ET-1)的关系。方法:缺血性脑血管病和心血管病及其相关组分诊断标准采用WHO及国家相关机构认可的工作定义。应用放射免疫分析,对93例缺血性脑血管病和103例冠心病患者进行了血清Ald和血浆ET-1的测定,与相同年龄对照组进行比较,并进行相关性分析。结果:①与正常人(62.52±10.40)pg/ml比较,ICVD和CHD各组Ald水平均显著增高,以高血压组(HBP)增高最为显著(89.69±30.11)pg/ml,两组Ald水平增高顺序为HBP〉椎基底动脉供血不足(VBI)〉急性脑梗死(ACI)〉冠心病(CHD)〉脑梗死后遗症(SCI)。②ET-1水平与对照组(58.25±12.2)pg/ml比较各组亦显著升高,HBP组ET-1为(80.29±18.35)pg/ml,相差极为显著(P〈0.01),其中HBP〉ACI〉CHD〉VBI〉SCI。③线性回归分析显示,两组患者Ald与ET-1之间均呈明显正相关关系,其中ICVD组93例患者,血清Ald与血浆ET-1(r=0.459,P〈0.01);HBP组Ald与ET-1(r=0.561,P〈0.01);CHD组Ald与ET-1(r=0.20,P〈0.05)。结论:Ald与ET-1与缺血性脑血管和心血管疾病的发生、发展密切相关,其浓度变化可作为评价心脑血管疾病时血管受损程度的客观指标。

Relationship Between Serum Aldosterone and Plasma Endothelin-1 Levels in Patients with Ischemic Cerebrovascular Disease or Coronary Heart Disease
Abstract:Objective To observe the relationship between serum aldosterone(Ald) and plasma endothelin-1 (ET-1) levels in patients with ischemic cerebrovascular disease(ICVD) or coronary heart disease(CHD). Methods The diagnostic criteria for ICVD and CHD were baced upon WHO and related national approved definitions. Serum Aid and plasma ET-1 levels were detected with radio immunoassay in 93 ICVD patients, 103 CHD patients, and 42 controls. Results ①Aid level was significantly higher in all ICVD and CHD patients with highest levels in high blood pressure (HBP) group. Ald level increased in the order: HBP 〉 Vertebrobasilar insufficiency (VBI)〉acute cerbral infarction (ACI)〉CHD〉sequela of cerebral infarction(SCI).②ET-1 levels were also significandy higher in all ICVD and CHD patients. ET - 1 level increased in the order: HBP〉ACI〉CHD〉VBI〉SCI.③Linear regression analysis showed that Aid and ET - 1 levels were positivly correlated in ICVD patients (r=0.459, P〈0.01), also in HBP(r=0.561, P 〈 0.01 ), and CHD ( r = 0.20,P 〈 0.05) patients. Conclusion Aid and ET-1 played important roles in the development of ICVD and CHD. Aid and ET - 1 may serve as parameters in evaluation of the damage of cardia - cerebrovascular disease.
