玉屏风颗粒对支气管哮喘患儿Th1／Th2免疫平衡的影响

目的探讨玉屏风颗粒对支气管哮喘的疗效及对患儿Th1／Th2免疫平衡的影响。方法将132例非急性发作期轻、中度支气管哮喘患儿随机分为观察组及对照组，两组均予布地奈德气雾剂吸入，每次200μg，每日2次。观察组另予口服玉屏风颗粒，6—9岁患儿每次5．0g，每天2次；10～14岁患儿每次5．0g，每天3次；每个疗程为1个月，每个疗程完成后停1周再进行另1个疗程，共进行5个疗程（6个月）。6个月后观察两组治疗效果及外周血Th1、Th2及血清白细胞介素4（IL-4）、γ-干扰素（IFN-γ）表达水平。结果观察组和对照组患儿哮喘控制率分别为86．7％和68％，两组临床控制率比较差异有统计学意义（x^2=5．57，P〈0．05）。治疗前两组Th1、Th2及Th1／Th2比较差异无统计学意义；治疗后两组Th1均升高、Th2均下降，Th1／Th2均升高，但观察组改变（Th1升高、Th2下降，Th1／Th2升高）更明显，两组比较差异有统计学意义（P〈0．05）；两组治疗后血清IL4含量均下降，但观察组下降更明显，两组比较差异有统计学意义（P〈0．05）；与治疗前比较，两组治疗后血清INF-1含量均升高，但观察组升高更明显，两组比较差异有统计学意义（P〈0．05）。结论玉屏风颗粒能提高支气管哮喘的治疗效果，改善支气管哮喘患儿Th1／Th2免疫失衡及血清IL-4、IFN-γ的表达水平，值得临床广泛推广。

Effect of Yupingfeng particles on immunologic balance of Th1/Th2 in children with bronchial asthma

Objective To investigate the clinical efficacy of Yupingfeng asthma and immunologic balance of helper T lymphocyte 1/helper T lymphocyte particles on bronchial 2（Th1/Th2）. Meth-ods One hundred and thirty-two children with non-acute stage mild to moderate bronchial asthma were randomly divided into observation group and control group, the patients in the two groups were all treated with Budesonide aerosol, 200 μg per time, 2 times a day;the patients in the observation group were addi-tionally treated with Yupingfeng particles, for age from 6 to 9, 5.0 g per time, 2 times a day; for age from 10 to 14, 5.0 g per time, 3 times a day, the course of therapy was one month, after one course, there was 1 week of suspension, and then proceed to the next course, until the fifth course was comple-ted. After 6 months, the clinical efficacy of the two groups was observed, the expression levels of Thl, Th2 were measured, and the levels of serum interleukin 4 （IL-4）, interferon γ（IFN-γ） were detected. Results The clinical total effective rate in the observation group（86. 7% ） was higher than that in the control group（68% ）, there was significant difference between the two groups（x^2 =5.57, P 〈0. 05）; the levels of Thl in the two groups all increased, the levels of Th2 in the two groups all decreased, Th1/ Th2 all increased, but the observation group （Thl increased, Th2 decreased, Th1/Th2 increased ） changed more significantly, there were significant differences between the two groups （ P 〈 0. 05 ） ; the levels of IL-4 in serum decreased after the treatment in the two groups, but the observation group de-creased more obviously, there were significant differences between the two groups （ P 〈 0. 05 ） ; compared with before treatment in the two groups, the levels of INF- γ increased, but the observation group in- creased more obviously, there were significant differences between the two groups. Conclusions Yup-ingfeng particles is effective on bronchial asthma by adjusting immune imbalance of Th1/Th2 and the ex-pression levels of serum IL-4, IFN-γ, is worthy of clinical application.
