经腹根治性全胃切除术治疗老年贲门癌患者疗效观察

目的探讨经腹根治性全胃切除术治疗老年贲门癌的疗效和应用价值。方法将60例老年贲门癌患者根据随机数字表法分为观察组和对照组，观察组30例患者行经腹根治性全胃切除术，对照组30例患者行近侧胃大部切除术，比较两组术后并发症发生率和术后生存率。结果观察组术后。并发症发生率为13．3％，低于对照组的40．0％，差异有统计学意义（P〈0．05）；观察组术后1、3、5年的生存率分别为83．3％、60．0％及40．O％；对照组术后1、3、5年的生存率分别为80．0％、43．3％及20．0％。两组术后1年的生存率比较差异无统计学意义（P〉0．05），术后3年及5年的生存率比较差异有统计学意义（P〈0．05）。结论经腹根治性全胃切除术治疗老年贲门癌能显著提高术后患者生存率，且并发症发生率较低，故可用于手术治疗老年贲门癌，值得在临床上进一步推广。

Efficacy of transabdominal radical total gastrectomy on elderly patients with cardiac cancer

Objective To investigate the efficacy of transabdominal radical total gastrectomy on elderly patients with cardiac cancer. Methods Sixty elderly cardiac patients were randomly divided into observation group and control group, 30 patients in observation group were treated by abdominal radical to- tal gastrectomy, 30 patients in control group were treated by proximal gastrectomy, postoperative disease in- cidence and survival rates were compared. Results Postoperative complication rate of the observation group was 13.3%, which was lower than the 40.0% in the control group, the difference was statistically significant （P 〈 0.05） ; After 1 year, 3-year and 5-year survival rates of the observation group were 83.3 %, 60.0% and 40.0% ; After 1 year, 3-year and 5-year survival rates of control group were 80.0%, 43.3% and 20. 0%. There was no significant difference between the two groups in 1 year postoperative sur- vival rates（P 〉 0.05）, there were significantly differences between the two groups in 3-year and 5-year postoperative survival rates （P 〈 0. 05 ）. Conclusions Transabdominal radical total gastrectomy senile car- diac surgery can significantly improve the survival rate of patients, and the incidence of complications is low, it can be used for treatment of senile cardiac surgery, and be worthy of clinical further promotion.
