三维造影磁共振血管扫描在脑动静脉畸形诊断和手术中的应用

摘    要:目的 探讨三维造影磁共振血管扫描(3DMRA)对脑动静脉畸形(AVM)的诊断作用及在AVM摘除术中的应用价值。方法 对6名不同部位脑AVM病例(运动区近旁AVM2例,运动性语言中枢近旁、感觉性语言中枢近旁、颞叶、枕叶AVM各1例)采用GE Signa Horizon 1.5T核磁共振扫描机,应用对比剂Gd—DTPA进行三维SPGR摄影,将制作出的3DMRA图像与脑血管造影(DSA)及手术中所见进行对比,判断该方法和制作出的影像在脑AVM诊断和手术中的嘘用价值。结果 6例脑AVM患者的3DMRA图像均较清楚地显示出脑表面供血动脉、病灶及引流静脉与脑组织的相互位置关系,并可以显示出供血动脉进入脑组织和引流静脉流出脑表向的位置。影像所见与术中所见一致。结论 3DMRA可直观地显示AVM的供血动脉、病灶及引流静脉与脑组织的关系,为AVM的做创手术治疗提供有益的影像依据。

Abstract:Objective To evaluate the clinical significance of enhanced three-dimensional magnetic resonance angiography (3DMRA) in diagnosis and resection of intracranial arteriovenous malformation (AVM). Methods 6 patients with AVM were performed 3DMRA examinations by GE Signa Horizon 1.5T. The 3DMRA images were compared with of DSA images and intro-operative finding.Results 3DMRA could demonstrate the feeding arteries, nidus and draining veins of AVM.The relationship of the AVM and brain tissue was showed steroscopically. The 3DMRA images and intra-operative finding were same or idential. Conclusions 3DMRA could demonstrate the AVM steroscopically, It provided useful informations in minimally invaste surgery for AVM.
