椎管内分娩镇痛对新生儿缺血缺氧性脑病的影响

目的观察椎管内分娩镇痛对新生儿缺血缺氧性脑病(HIE)的影响。方法选择2018年1月-2019年4月新疆哈密第十三师红星医院收治的足月单胎头位初产妇患者100例,分为无痛分娩组和常规分娩组各50例。无痛分娩组采用舒芬太尼配伍罗哌卡因椎管内分娩镇痛,常规分娩组未采取任何镇痛措施。观察两种分娩方式对产妇及新生儿的影响。结果两组新生儿娩出后脐动脉血气分析、血电解质、血糖检测,新生儿脑电图、脑干听觉诱发电位检查及新生儿神经行为评分差异均无统计学意义(P>0.05);常规分娩组剖宫产中转率为10.0%,高于无痛分娩组的4.0%,差异有统计学意义(P<0.05)。结论舒芬太尼配伍罗哌卡因椎管内分娩镇痛对新生儿有较高的安全性,且不会增加新生儿HIE的发生,值得临床推广应用。

Influence of spinal block for labor analgesia on hypoxic ischemic encephalopathy

Objective To investigate the influence of spinal block for labor analgesia on neonatal hypoxic ischemic encephalopathy(HIE). Methods From January 2018 to April 2019, 100 full term primiparas with single-fetal cephalic presentation were selected from the Red Star Hospital of the 13 th Division of Hami, Xinjiang. They were divided into painless delivery group and routine delivery group with 50 cases each.Sufentanil combined with ropivacaine for spinal block labor analgesia was used in painless delivery group,while no analgesic measures were taken in routine delivery group. The effects of two delivery methods on the parturient women and newborns were observed. Results T here were no significant differences on blood gas analysis of umbilical artery,blood electrolyte and blood sugar detection,EEG,BAEP test and NBNA score of two groups(P>0.05). The conversion rate of cesarean section in routine delivery group was 10.0%,which was higher than 4.0% in painless delivery group,the difference was statistically significant(P <0.05). Conclusion Sufentanil combined with ropivacaine for intraspinal delivery analgesia had higher safety for neonates, and did not increase the incidence of neonatal HIE,which was worthy of clinical promotion and appliation.
