肠梗阻导管联合生长抑素治疗肠梗阻的研究进展

肠梗阻是一种常见的外科急腹症,手术治疗容易出现各种并发症,因此非手术治疗和术前准备显得尤为重要。近年来,随着肠梗阻导管材质的进步,其被越来越多地应用于临床,例如治疗单纯性肠梗阻、绞窄性肠梗阻、小肠排列术、小肠瘘、术后早期粘连性肠梗阻以及恶性肠梗阻的术前辅助治疗,其肠腔减压效果明显优于常规胃管减压。生长抑素可以抑制胃泌素、胃酸、胰岛素以及胃蛋白酶的分泌,降低胰腺的内外分泌,减少胃、小肠以及胆囊等消化液的分泌,降低肠腔压力,减轻肠管扩张,从而改善肠壁血液循环,促进肠功能恢复。肠梗阻导管联合生长抑素在各类型肠梗阻的治疗中具有良好疗效。本文对肠梗阻导管的适应证、置管方法、生长抑素用法、药理作用进行综述。

Research progress in the treatment of intestinal obstruction by intestinal obstruction catheter combined with somatostatin

The intestinal obstruction is a common surgical acute abdomen. It is easy to have various complications in surgical treatment, so non-surgical treatment and preoperative preparation are particularly important.In recent years, with the progress of intestinal obstruction catheter material, it is more and more used in clinic, such as the treatment of simple intestinal obstruction, stangulated intestinal obstruction, small intestinal arrangement, small intestinal fistula, early postoperative adhesive intestinal obstruction and preoperative adjuvant treatment of malignant intestinal obstruction. The effect of intestinal decompression is significantly better than that of conventional gastric tube decompression. Somatostatin can inhibit the secretion of gastrin, gastric acid, insulin and pepsin, reduce the internal and external secretion of pancreas, reduce the secretion of digestive fluid such as stomach, small intestine and gallbladder, reduce the pressure of intestinal cavity, reduce the dilatation of intestinal canal, so as to improve the blood circulation of intestinal wall and promote the recovery of intestinal function. The combination of intestinal obstruction catheter and somatostatin has a good effect in the treatment of various types of intestinal obstruction. This article reviews the indications, catheterization methods, somatostatin usage and pharmacological effects of intestinal obstruction catheter.
