高清内镜结合电子染色对慢性萎缩性胃炎的诊断价值

目的探讨高清内镜结合电子染色（i-scan）在萎缩性胃炎诊断中的价值。方法将343例患者随机分为3组,分别应用普通内镜检查、高清内镜普通白光模式、高清内镜i-scan模式检查,根据肉眼所见管腔黏膜的变化,判断是否有胃黏膜萎缩,取黏膜做病理检查,比较病理组织学结果。结果普通内镜检查100例,诊断萎缩性胃炎54例,诊断率为54%;高清内镜普通白光模式检查114例,诊断萎缩性胃炎83例,诊断率为72.8%;高清内镜i-scan模式检查129例,诊断萎缩性胃炎110例,诊断率为85.27%。与病理组织学结果比较,普通内镜、高清内镜普通白光模式、高清内镜i-scan模式诊断萎缩性胃炎的敏感度和特异度分别为70.37%和76.09%;74.70%和80.65%;78.18%和84.21%。结论高清内镜iscan模式与普通内镜、高清内镜白光模式相比更容易发现胃黏膜萎缩的病变位置,有助于萎缩性胃炎的检出。

HD endoscopy with i-scan mode in diagnose of chronic atrophic gastritis

Objective To evaluate the application of HD endoscopy with i-scan mode in diagnosis of atrophic gastritis. Methods Total 343 patients undergoing endoscopic examination w ere randomly divided into three groups： 100 cases w ere examined w ith conventional endoscopy（ group A）,114 w ith HD endoscopy using w hite mode（ group B） and 129 cases w ith HD endoscopy using i-scan mode（ group C）. The mucosa samples taken from biopsy were examined pathologically and the histopathological results w ere compared. Results The diagnostic rate of atrophic gastritis in groups A, B and C w ere 54. 0%（ 54 /100）,72. 8%（ 83 /114） and 85. 3%（ 110 /129）,respectively. With histopathology results as the gold standard for diagnosis of atrophy gastritis,the sensitivity of groups A,B and C w as 70. 37%,74. 70%,and 78. 18%; the specificity w as 76. 09%,80. 65% and 84. 21%,respectively. Conclusion The HD endoscopy w ith i-scan mode is more accurate than conventional endoscopy in diagnosis of gastritis atrophy.
