经尿道前列腺电切术后及早拔出留置尿管的效果观察

目的 探讨经尿道前列腺电切术(transurtbral resection of prostate,TURP)后及早拔除留置尿管的效果.方法 将198例患者随机分为两组.实验组患者术后1～3 d拔除留置尿管.对照组术后5～7d拔除留置尿管,观察两组患者拔除留置尿管后尿路感染、尿道狭窄、膀胱痉挛、继发性出血的情况.结果 实验组泌尿系统总并发症的发生率为6.06％,对照组为20.20％(x2=8.68,P＜0.05)；实验组平均住院费用为( 10584.00±401.23)元,对照组平均住院费用为(12138.20±621.45)元(t=-20.91,P＜0.05)；实验组平均住院日为(7.80±2.20)d,对照组平均住院日为(13.20±3.58)d(t=-12.79,P＜ 0.05).结论 及早拔除导尿管不仅可以减少术后泌尿系的并发症,而且还可以降低住院费用,缩短平均住院日,提高患者对护理工作的满意度.

Effect of early withdrawal of detained urinary catheter after transurethral resection of prostate

Objective To explore the effect of early withdrawal after transurethral resection of prostate （TURP）. Methods 198 patients were randomly divided into the experiment group and the control group. In the former group, the detained urinary catheters were withdrawn within 1 - 3 days after the resection, while in the control group the catheters in 5 - 7 days after the resection. The two groups were compared in respect of infection incidences of urinary catheter, urethral stricture, bladder spasms and secondary hemorrhage. Results The total incidences of complications in urinary system in the experiment group and the control group were 6.06% and 20.20%, respectively （X2 = 8.68, P 〈 0.05）. The average hospitalization expenses were （10584.00 ± 401.23） and （12138.20 ±621.45）, （t = -20.91, P 〈 0.05）. The average hospitalization durations were （7.80 ±2.20） and （13.20 ±3.58）, （t = -12.79, P 〈 0.05）. Conclusion Early withdrawal of urethral catheter can not on]y reduce the rate of complications in urinary system, but also shorten the average duration of hospitalization, lower the hospitalization expenses and improve patients＇ satisfaction with nursing service.
