体表超声在胃癌定量诊断中的应用

目的 :探讨体表超声在胃癌定量诊断中的应用价值及临床意义。方法 :对 5 1例胃癌患者进行术前超声检查并与手术病理进行对比分析。结果 :B超胃癌定量诊断 (浸润深度、浸润范围 )与术后病理的符合率分别为 80 .4%、86 .2 %。其中对浸润深度 T1 ～ T4各期判断符合率分别为 6 0 %、87.7%、81.4%、10 0 % ;对胃壁浸润范围 (上下边界距离 )小于 7.0 cm和大于 7.0 cm及弥漫浸润型胃癌 (皮革胃 )判断符合率分别为 87.5 %、81.2 %、10 0 %。结论 :超声对胃癌的定量诊断为胃癌术前分期提供了重要依据 ,对临床选择治疗方案和手术切除范围具有一定的指导意义。

Clinical Application of Body Surface Ultrasonography in the Quantitative Diagnosis of Gastric Carcinoma

Objective:To investigate clinical value of body surface ultrasonography(BSU)inthe quantitative diagnosis(QD)of gastric carcinoma(GC).Methods:To analyse 51 cases in ultrasonographic examination before operation comparatively with pathological diagnosis after operation.Results:The QD of GC(eroded depth and range)in ultrasonography was compared with pathological diagnosis after operation,and their coincidence rate was 80 4% and 86 2% respectively.The diagnostic coincidence rate of the eroded depth in the four stages,T 1 to T 4,was 60%,87 7%,81 4% and 100% respectively.The coincidence rate of infiltrative range(distance of the stomach wall from intima to serosa)less or larger than 70mm and diffuse infiltrative(the Borrmawn IV type.leather bottle stomach)was 88 5%,84 6% and 100% respectively.Conclusions:QD ultrasonography provides important basis for dividing different phases of cases with GC before operation.It has certain clinical guiding effects in chosing methods of treatment and surgical excision range.
