五紫平喘汤对ACCOPD伴喘息患者症状缓解及炎症因子水平的影响

目的：观察五紫平喘汤对慢性阻塞性肺疾病（ACCOPD）伴喘息临床症状缓解及炎症因子水平调节效果。方法将72例住院或门诊的 ACCOPD 伴喘息患者随机分为中西医组（ n=36）和西医组（ n=36）；西医组给予西药平喘治疗，中西医组给予西药平喘和我院自制五紫平喘汤治疗。治疗后比较两组临床咳、痰、气急、喘息症状缓解效果，检测并比较两组炎症因子[肿瘤坏死因子（TNF-α）、白介素-8（IL-8）、C 反应蛋白（CRP）及α-酸性糖蛋白（AAG）]水平，并评估安全性。结果两组治疗前临床主要症状积分与相关炎症因子水平比较无统计学意义（P＞0.05），与治疗前比较，两组治疗后四项临床主要症状积分及炎症因子水平均明显降低（P＜0.01）；但与西医组比较，治疗后中西医组咳、痰、气急、喘息积分显著降低（P＜0.05），治疗后中西医组 TNF-α、1L-8、CRP、AAG 均显著降低（P＜0.05）；两组均未出现明显的不良反应或毒副作用。结论在常规西药治疗基础上加用五紫平喘汤治疗，可显著缓解 ACCOPD 伴喘息患者临床咳、痰、气急、喘息等症状，改善血清炎症因子水平，安全性高，推荐推广使用。

Effects of Wuzi Pingchuan Decoction on Symptoms Relieving and Levels of Inflammatory Factors in Patietns with ACCOPD Complicated with Asthma

Objective To observe the effects of Wuzi Pingchuan Decoction on relieving clinical symptoms in patients with chronic obstructive pulmonary disease (ACCOPD) complicated with asthma and regulating inflammatory factors. Methods Seventy-two patients with ACCOPD complicated with asthma in our hospital were randomly divided into traditional Chinese and Western medicine group (n=36) and Western medicine group (n=36). The Western medicine group was treated with Western medicine for relieving asthma while the traditional Chinese medicine and Western medicine group was treated with Western medicine for relieving asthma combined with self-made Wuzi Pingchuan Decoction. After treatment, the relief of cough, phlegm, dyspnea and wheezing symptoms and pulmonary function grading were compared between the two groups. The levels of related inflammatory factors [tumor necrosis factor alpha (TNF-α), interleukin-8 (IL-8), CRP (creactive protein) and alpha acid glycoprotein (AAG)] were detected and compared between the two groups before and after treatment and safety of treatment was evaluated. Results There were no significant differences in integral of main clinical symptoms, pulmonary function grading and levels of related inflammatory factors between the two groups before treatment (P>0.05). After treatment, the integral of four main clinical symptoms and levels of related inflammatory factors were significantly lower than those before treatment (P<0.01). Compared with Western medicine group, the scores of cough, sputum, dyspnea and wheezing in the traditional Chinese medicine and Western medicine group were significantly lower (P<0.05); After treatment, the levels of TNF-α, IL-8, CRP and AAG in the traditional Chinese medicine and Western medicine group were significantly lower (P<0.05); There were no obvious adverse reactions or side effects in the two groups. Conclusion Routine Western medicine combined with Wuzi Pingchuan Decoction could significantly relieve the clinical symptoms such as cough, phlegm, dyspnea and wheezes, and improve the levels of serum inflammatory factors in patients with ACCOPD complicated with asthma, with high safety, and worthy of popularization.
