青壮年无神经损伤胸腰椎爆裂骨折的手术治疗

目的探讨手术治疗青壮年无神经损伤胸腰椎爆裂骨折的临床价值。方法对30例50岁以下成人无神经损伤胸腰椎爆裂骨折患者施行复位内固定，经伤椎椎弓根植骨同时后路椎板间植骨。结果随访4个月～12个月，功能恢复良好，影像学显示脊柱无后凸畸形，恢复的椎体高度无下降，亦无腰背痛发生。结论手术治疗青壮年无神经损伤胸腰椎爆裂骨折，能最大限度地恢复脊柱三维空间结构和生物力学状态，能有效地防止脊柱骨折后遗症的产生。

Surgical treatment of the thoracolumbar burst fracture without neural impairment in younger patients

Objective To explore the clinical value of the surgical treatment of the thoracolumbar burst fracture without neural impairment in younger patients. Methods 30 patients less than fifty years old were treated by open reduction and internal fixation （ORIF）.Interlaminar fusion and bone graft into fractured vertebral body through pedictes were performed at the same time. Results The patients has been followed up from 4 to 12 months and good function was achieved in all cases. Thoracolumbar kyphosis was corrected after operation. Significant decrease has not been found during the follow-up period.Conclusion ORIF and bone graft is ideal treatment for thoracolumbar burst fracture without neural impairment in youth, which can be effectively used to restore the 3-D construction and the biomechanical state of the vertebrae. It will effectively prevent the occurrence of the sequela of the thoraeolumbar burst fracture.
