儿童脾海绵状血管瘤23例报告

目的　了解脾海绵状血管瘤患儿的临床特点及诊治要点。方法　总结了 1997～ 2 0 0 3年深圳市儿童医院和 1980～ 2 0 0 3年中南大学湘雅二院收治的 2 3例脾海绵状血管瘤患儿的临床表现、实验室检查、影像学检查、病理学检查的特点及其治疗经过和预后情况。结果　脾海绵状血管瘤患儿的主要临床表现为腹部包块、腹痛、腹部不适 ,B超多表现为增强回声 ,彩色多普勒多显示其内部有静脉血流信号 ,CT平扫多表现为均质的低密度灶 ,增强扫描多表现为均质性显著强化。脾海绵状血管瘤患儿进行脾切除术效果良好。结论　脾海绵状血管瘤的诊断主要依靠影像学检查 ,治疗可根据临床表现的有无及时或择期行脾切除术等干预措施。

Cavernous hemangioma of spleen in children:a report of 23 cases

Objective To summarize the clinical characteristics and key points in diagnosis and treatment of cavernous hemangioma of spleen in children.Methods Clinical feature,laboratory test,imaging and pathological examination of 23 children with cavernous hemangioma of spleen was reviewed,as well as the treatment and prognosis of them.Results Major clinical presentations of cavernous hemangioma of spleen were abdominal mass,abdominal pain or discomfort.Major sonographic findings were splenomegaly with increased echogenic foci of varied sizes,in which signal of venous flow was found by color Doppler.Major CT findings were splenomegaly with scattered homogenous low-density nodules which demonstrated homogenous enhancement under enhanced CT.Splenectomy had good therapeutic effect.Conclusion The diagnosis of cavernous hemangioma of spleen relies on imaging findings,and its treatment can be timely or time-selected splenectomy and other interventions according to the clinical manifestations.
