结肠水疗与常规灌肠对混合痔术前准备效果对比研究

目的:探讨对混合痔手术患者采用结肠水疗与常规术前肠道准备的效果。方法:选择280例混合痔择期手术患者,随机分为对照组和观察组各140例。观察组采用DJS-B型结肠水疗仪,于术前下午反复灌洗10～15次;对照组应用生理盐水500ml,于术前晚20:00和术日晨6:00各清洁灌肠5～6次和2～3次。评估患者对灌肠舒适满意程度,术中观察肠道清洁程度(手术医师反馈)、灌洗时间以及术后24h控制排便情况等。结果:平均每例灌洗时间对照组和观察组分别为(98±16)min和(45±7)min;肠道清洁程度:对照组优良率34%,观察组优良率90%;灌肠的舒适度(耐受情况):对照组和观察组分别为68%和96%。对照组控制排便时间为(18±6)h,观察组为(48±6)h,两组比较,观察组在各方面均优于对照组(P0.05)。结论:结肠水疗与常规灌肠比较,术前采用结肠水疗方法行肠道清洁,不仅具有简便、省时、安全、舒适、清洁更彻底,无不良反应,相对减轻护士工作的烦锁及工作量等优点,而且术后大便控制24h以上,减少了切口污染。

Study on the effects of intestinal hydrotherapy and the conventional enema on mixed hemorrhoid preoperative preparation.

Objective：To assess the different effects of intestinal hydrotherapy and the conventional enema on mixed hemorrhoid preoperative preparation.Methods：280 patients undergoing elective operation were randomly divided into control group and observation group,with 140 patients in each group.The observation group were subjected to enema repeatedly 10 to 15 times with type DJS-B intestinal hydrotherapy instrument.It’s took about 40-50 min（average 45min）in the afternoon just before the operation day.Group A were subjected to enema with 500 ml 0.9% NaCl hydro-solution 5 to 6 times at 8pm before the operation day,and 2 to 3 times at 6am of the operation day.The enema comfortable with satisfaction,intraoperative observation intestinal cleanliness and control defecate time after operation were assessed.Results：The average enema time for each case was （98±16） min in control group,and （45±7） in observation group.The intestinal sanitary degree was 34% in control group and 90% in observation group.Enema comfort（tolerance of enema）degree was 68% in control group and 96% in group B.The control defecate time for each case was（18±6） h in control group and （48±6） h in observation group.Conclusion：Compared with the conventional enema the preoperative intestinal hydrotherapy is a simple,savetime,safe,comfortable,clean procedure.It has litter adverse reaction,besides it can reduce nurse worklord and through the control defecate time reduce wound infection.
