多层螺旋CT在团注法增强的胰腺成像中的评价

摘    要:目的 探讨胰腺多层螺旋CT团注法增强的最佳时像和扫描方案。方法 选取19例正常胰腺和23例胰腺癌病人,均行团注法增强扫描,比较胰腺组织在动脉期、胰腺期和门静脉期的强化程度、肿瘤同瘤旁正常胰腺组织的CT差值。结果 正常胰腺在胰腺期强化最显著,高于动脉期和门静脉期(P〈0.05),动脉期和门静脉期的强化程度相比较无明显差别(P〉0.05);胰腺癌组三个时期的肿瘤同瘤旁正常胰腺组织的CT差值相比较中,胰腺期的CT差值最大(P〈0.01),动脉期和门静脉期的CT差值相比较无差别(P〉0.05)。结论 胰腺团注法增强CT扫描中的胰腺期反映胰腺强化程度最佳,此期对胰腺癌的检出效果优于动脉期和门静脉期。

Abstract:Objective To investigate the optimal temporal window and scanning scheme for pancreatic dynamic bolus enhanced with multi-slice helical CT scanning. Methods Dynamic bolus enhanced scanning of pancreas with MSCT were performed in 19 patients without pancreatic lesions and 23 with pancreatic carcinoma patient. And compared the enhancement degree of pancreatic parenchyma between arterial phase, pancreatic phase and portal phase, the value between the lesions and normal pancreas parenchyma was calculated. Results Enhancement value of normal pancreas was most prominent during pancreatic phase, than arterial phase and portal phase ( P <0.05), no obvious difference was found statistically between arterial phase and portal phase ( P >0.05). In pancreatic carcinoma group, the|? value between the lesions and normal pancreas parenchyma during pancreatic phase was the greatest of all phase ( P <0.01), the difference was not found between arterial phase and portal phase ( P >0.05). Conclusion Pancreatic phase of dynamic bolus enhanced scanning of pancreas with MSCT was optimal temporal window in reflecting enhancement degree of pancreatic parenchyma, and had an advantage in finding out the lesions of pancreatic carcinoma than during arterial phase and portal phase.
