半椎板入路显微手术切除椎管内神经鞘瘤

目的探讨经半椎板入路切除椎管内神经鞘瘤的应用价值。方法采用一侧半椎板入路显微切除椎管内神经鞘瘤22例，其中肿瘤位于颈段10例，胸段7例，腰段5例。结果所有患者肿瘤均被全切除，术后症状明显改善，随访期间未见新的神经功能损害或脊柱畸形并发症。结论半椎板入路显微手术切除椎管内神经鞘瘤具有手术创伤小、恢复快且有利于维持脊柱稳定性的优点，适用于治疗位于侧面或侧背面生长的神经鞘瘤。

Microsurgery Through Unilateral Hemilaminectomy Approach for the Treatment of Intravertebral Schwannomas

Objective To investigate the application of hemilaminectomy approach to the microsurgery for intravertebral canal schwannomas. Methods Twenty-two patients with intravertebral canal schwannomas underwent mierosurgery through the unilateral hemilaminectomy, including ten cases of the schwannomas originated from cervical, seven eases from thoracic, five cases from lumbar. Results Postoperative MR images demonstrated complete tumor removal in all patients. Symptoms and signs were sig- nificantly improved after treatment, and none of new neurological dysfunction was found in all patients after the operation as well as spinal abnormality during follow-up. Conclusion Mierosurgery through unilateral hemilaminectomy approach for intravertebral canal schwannomas had merits including minimal invasiveness and rapid recovery and the ability to keep the spinal stability, which can be used in the treatment of schwannomas both unilateral and doral to the cord.
