急性脑梗死患者血清抗心磷脂抗体检测的临床意义

目的探讨抗心磷脂抗体（antieardiolipin antibodies，ACA）与急性脑梗死（acute cerebral infarct，ACI）的关系及其，临床意义，为临床早期预防性干预提供依据。方法采用酶联免疫吸附法（ELISA）检测69例ACI患者ACA的阳性率，并与对照组42例的检测结果进行比较。结果ACI组患者ACA阳性率24．64％，对照组ACA阳性率4．76％，两组相比差异有统计学意义（P〈0．01）；ACI组患者年龄≤50岁的ACA阳性率36．11％，年龄〉50岁的ACA阳性率12．12％，ACI组患者ACA阳性率与年龄≤50岁脑梗死患者关系密切（P〈0．05）；而ACI组患者ACA的阳性率在性别差异上无统计学意义（P〉0．05）；ACI组患者多灶性脑梗死ACA阳性率42．86％，单灶性脑梗死ACA阳性率16．67％，ACA阳性率与脑梗死病灶数量呈正相关，两者比较差异有统计学意义（P〈0．05）。结论ACA是ACI，尤其是中、青年ACI的重要危险因素，且以多灶性脑梗死多见，临床上应重视对ACA阳性患者的积极防治。

Detection of Serum AnticardioUpin Antibodies in Patients with Acute Cerebral Infarction and its Clinical Significance

Objective To explore the relations between the level of anticardiolipin antibodies （ACA） and acute cerebral infarct （ACI） , provide reference for the early clinical prevention and intervention of ACI. Methods The serum levels of ACA of 69 cases of ACI and 42 control subjects were assayed with ELISA methods. Results The positive rate of ACA in ACI group was sig- nificantly higher than that in the control group （24.64% vs 4.76%, P 〈 0.01 ） ;In the ACI group,36.11% of patients under 50 years of age were positive for serum ACA against 12.12% of patients with more than 50 years, ACA positive rate was closely related with the younger age（P 〈 0.05 ） ;but not with gender（ P 〉 0.05 ）. ACA positive rate in the ACI patients with multi-focus infarction was 42.86%, against 16.67% in patients with single focus cerebral infarction （P 〈 0.05 ）. Conclusion ACA is an important risk factor in the development of ACI, particularly for middle-aged patients with multi-focus infarction. ACA positive patients should be paid more attention in the clinical treatment and prevention.
