西咪替丁预防脑出血并发应激性溃疡出血

目的：观察西咪替丁静脉滴注治疗高血压性脑出血并发应激性溃疡出血的疗效。方法：将１２６例临床分期为Ⅲ－Ⅳ级的高血压性脑出血病人在常规脱水利尿、对症支持治疗的基础上，随机分为西咪替丁组（男性 ４３例，女性 ２０例；年龄 ６０ ａ± ｓ１１ａ）和对照组（男性 ４６例，女性 １７例；年龄 ５９ａ±１２ ａ）各６３例。西咪替丁组加用西咪替丁 ０．４ ｇ， ｉｖｇｔｔ， ｂｉｄ × １４ ｄ。结果： ２组应激性溃疡出血的发生率分别为 ６％和 ３３％，脑出血死亡率为 ８％和 ３２％，西咪替丁组显著低于对照组（Ｐ＜０．０１）。结论：西咪替丁静滴预防高血压性脑出血并发应激性溃疡出血疗效确切，且无明显不良反应。

Cimetidine in preventing stress ulcer hemorrhage after cerebral hemorrhage

AIM: To observe the efficacy of cimetidine prevent hypertensive cerebral hemorrhage complicated with stress ulcer hemorrhage. METHODS: One hundred and twenty-six patients with cerebral hemorrhage accepted conventional therapy and they were randomly divided into cimetidine group (M 43, F 20, age 60 a± s 11 a) and control group (M 46, F 17, age 59 a± 12 a). Cimetidine group received cimetidine 0. 4 g, v drip bid × 14 d. RESULTS: The morbidity of stress ulcer hemorrhage was 6% in cimetidine group and 33% in control group (P < 0. ０１ ) the mortality of cerebral hemorrhage was 8% in cimetidine group and 32% in control group (P < 0. 01 ). There were no obviously adverse reactions. CONCLUSION: Intravenous drip of cimetidine is effective in preventing hypertensive cerebral hemorrhage complicated with stress ulcer hemorrhage.
