椎管内外生骨疣的X线、CT、MRI特征及其诊断

目的 :探讨椎管内外生骨疣的临床、影像学特征及诊治方法。材料和方法 :经手术及病理明确诊断的共 8例 ,男 ,5例 ,女 ,3例。年龄 2 1～ 65岁 ,平均 45岁。胸椎管 4例 ,腰椎管 2例 ,颈椎管 2例。对上述病例的的临床、影像学表现进行回顾性研究。结果 :临床上 ,上述病例均表现为进行性的双下肢或四肢的功能障碍 ,普通X线仅 2例发现可疑异常 ,全部病例CT或MRI检查均呈椎板向椎管内的带蒂状骨性突出。其致残率及致死率均较高 ,经手术治疗后均获得临床治愈 ,6例患者脊髓压迫症状获得不同程度的恢复 ,2例患者截瘫无改善。结论 :发生于椎管内的外生骨疣早期容易出现截瘫 ,手术是唯一有效的治疗方法。早期发现和早期手术可彻底治愈并挽救生命。

X-ray, CT and MRI in the Osteochondroma Grossing in Vertebrae Canal

Purpose: To probe into the clinical and imaging feature,diagnosis and therapy methods about the Osteochondroma grossing in vertebrae canal. Materials and Methods: The clinical and imaging data of 8 cases proved by operation and pathology were analyzed retrospectively(5 males, 3 females,mean age 45). Of 8 cases,4 cases suffured from thoratic canal, 2 cases lumbar canal and 2 cases cervical canal. Results: All patients presented aggressive extremities dysfunction. Abnormal lesions were seen only in 2 cases on X-ray film. The vertebrae exstosis occurred in all cases on CT and MRI. In this group, the mobility and mortality of this disease were high that 6 cases had some relieve, 2 cases hadn't any relieve after operation. Conclusion: Exostosis rarely occur in vertebrae canal, the patients in early stage present disability. Operation is only effective method of this disease. Early diagnosis and therapy of this disease is important of saving these patients.
