蠲痹汤调控淋巴管回流功能治疗膝骨关节炎的研究＊

目的 探讨蠲痹汤（JBT）对膝骨关节炎（KOA）小鼠模型淋巴管回流功能及关节病理退变的影响。方法 将120只C57BL小鼠随机分成JBT,莫比可（MOBIC）与PBS组。小鼠右膝接受内侧半月板失稳（Destabilization of medial meniscus,DMM）手术以诱导KOA模型,左膝作为假手术对照。使用近红外光谱检测系统评估关节淋巴管回流情况;ABHO染色评估关节软骨的面积及OARSI评分;免疫组织化学染色观察膝关节II型胶原含量及软骨细胞退变指标蛋白聚糖降解产物（Nitege）的积累。结果 与PBS、Mobic组比较,JBT组小鼠膝关节的ICG清除率在术后4周和8周明显增强,OARSI评分较低;JBT组关节软骨中Nitege的表达低于PBS组,但是II型胶原表达高于PBS组。结论 JBT可以促进早期KOA小鼠膝关节淋巴管回流功能,并且延缓KOA的病理进程。蠲痹汤是一种有效治疗膝骨关节炎的中医药经典方剂,其疗效机制可能是通过上调关节淋巴回流功能实现的。

Study on Juan-Bi-Tang in Treating Knee Osteoarthritis by Promote Draining Function of Lymphatic

Objective To study the effects of Juan-Bi-Tang (JBT) on joint lymphatic draining function and pathologic progress of KOA mouse model. Methods 120 C57BL mice were randomly divided into 3 groups (treated with JBT, Mobic or PBS). All of mice received the surgery of destabilization of medial meniscus (DMM) on the right knee to induce osteoarthritis model, and sham surgery on the left knee as control. We used near infrared detection system to assess joint lymphatic draining function; ABHO staining to evaluate the area of articular cartilage and OARSI Scores; immumohistochemical staining (collagen type II and NITEGE, the G1 fragment of the proteoglycan aggrecan) to detect joint cartilage degeneration. Results At 4 and 8 weeks after surgery, OARSI score of JBT group was lower and its joint ICG clearance was significantly enhanced compared with Mobic and PBS group; collagen type II expression was higher but Nitege expression was lower in JBT group compared with PBS group. Conclusion JBT can promote the joint lymphatic function and postpone pathologic progress in KOA mouse model. Therefore, JBT is an effective traditional Chinese medicine for KOA therapy, and its effective mechanism could be up-regulating joint lymphatic drainage.
