预构前臂皮瓣唇再造的适形设计和外科操作

目的探讨预构前臂皮瓣再造唇的形态设计和外科操作技巧。方法利用前臂皮瓣及其血管蒂作为载体,以口腔黏膜、腹部皮片作为嵌入成分,预构唇的解剖外形,经Ⅱ期显微外科移植再造唇。结果预构皮瓣所形成的唇获得天然器官的三维结构和外形,经显微外科移植和重建后均成活。结论与传统皮瓣相比,预构皮瓣能极大改观唇缺损修复重建的三维形态和美容效果。

Conformal prelamination and surgical procedures of prefabricated radial forearm flaps for reconstruction of lips

Objective To present the conformal prelamination and surgical procedures of prefabricated radial flaps for reconstruction of lips.Methods The forearm flap and its vascular pedicle were used as blood supply and carrier to which specially additional tissue(skin,mucous membrane)as chimera element was added to prefabricate the three-dimensional contour and morphology of lips.After a period of healing,the prefabricated flap was distantly transferred by microsurgical technique to reconstruct the lips.Results All the flaps survived and the contour and morphology of the reconstructed lips got an acceptable appearance.Conclusion Prefabricated flaps showed many advantages in reconstructing defects of organs such as enough time for surgeons to perform surgical preparation,sculptures and form the contour and imitate the natural appearance of the lips.It may improve the aesthetic outcomes after reconstruction to a great extent.
