银离子藻酸盐敷料在儿童四肢深II度烧伤创面中的应用效果观察

目的:探讨藻酸盐银离子敷料治疗儿童深II度烧伤创面中临床效果.方法:选取2015年7月—2016年5月我院烧伤整形科收治的四肢深II度烧伤患儿共60例,随机分为观察组和对照组.在创面清创后,观察组外用藻酸盐银离子敷料覆盖,无菌纱布包扎;对照组采用银锌霜皮肤黏膜抗菌剂涂抹,无菌纱布包扎.两组均根据创面渗出情况换药.比较两组患儿的换药次数、入院第7天的发热率、创面愈合率及愈合时间.结果:与对照组相比,观察组换药次数(8.63±2.37)次,明显少于对照组(14.70±2.30)次,差异有统计学意义(P<0.05).观察组治疗第7天患儿发热率为6.67%(2例),明显低于对照组23.33%(7例),差异有统计学意义(P<0.05);观察组治疗第7、11、15和19天创面愈合率明显高于对照组,差异有统计学意义(P<0.05);观察组创面愈合时间为(20.70±2.30)d,住院时间为(21.33±3.67)d,均较对照组(27.63±3.63)d和(28.30±3.30)d]缩短,差异具有统计学意义(P<0.05).结论:应用银离子藻酸盐敷料治疗儿童四肢深II度烧伤创面能减少换药次数,加速创面愈合,减少患者痛苦,值得推广和应用.

Observation on the effect of silver ion alginate dressing on deep II degree burn wounds on limbs of children

Objective: To investigate the clinical effects of silver ion alginate dressing on treatment of deep II degree burn wound of children.Methods: Sixty orthopaedic patients with deep II degree burns on limbs admitted to Bengbu the Third People's Hospital from July 2015 to May 2016 were randomly divided into observation group and control group (30 cases in each group). After debridement, the wound surface was covered with silver ion alginate dressing (observation group) or smeared with silver zinc cream antibacterial agent (control group), the wounds were then wrapped with sterile gauze. Fresh dressing was changed for according to the exudation of the wounds. The times of dressing-change, the fever rate 7-day after treatment, wound healing rate and healing time were compared between the two groups.Results: The times of dressing change were obviously less in observation group (8.63±2.37) than in control group (14.70±2.30) with statistical significance (P<0.05). The fever rate 7-day after treatment was obviously lower in observation group (2 cases, 6.67%) than in control group (7 cases, 23.33%) with significant difference (P<0.05). The wound healing rates were obviously higher in observation group than in control group on day 7, 11, 15 and 19 after treatment with statistical significance (P<0.05). The wound healing time and hospital stay were all shorter in observation group (20.70±2.30) days and (21.33±3.67) days, respectively] than in control group (27.63 ±3.63) days and (28.30±3.30) days, respectively] with statistical significance (P<0.05).Conclusions: Treatment with silver ion alginate dressings in children with deep II degree burn wounds may decrease the times of dressing-change, promote wound healing, reduce the patients' pain, so is worth to be popularized and applied.
