联合检测preS1和HBcAg在慢性乙肝患者中的临床价值

目的：探讨联合检测preS1和HBcAg（均为HBV核酸相关抗原，nucleic acids related antigen，HBVNRAg）在慢性乙肝患者诊疗过程中的临床价值。方法：对568例慢性乙型肝炎患者血清及486例正常对照血清采用实时荧光定量PCR（FQ—PCR）进行DNA定量检测，酶联免疫吸附试验（ELISA）进行preS1和HBcAg（结果以HBVNRAg表示）的联合检测，同时检测HBV DNA、HBV—M、ALT、AST。结果：在568例慢性乙肝患者的血清标本中，HBV NRAg检出率为98．3％，HBeAg的检出率为37．4％，前者明显高于后者，差异有统计学意义（P〈0．05）。1054份血清标本中，HBVNRAg阳性组较阴性组，ALT、AST检出率明显升高，差异有统计学意义（P〈0．05）。结论：联合检测preS1和HBcAg的HBVNRAg在筛选HBV感染及判断HBV复制方面具有较高的临床价值。

The clinical value of joint detection preS1 and HBcAg in patients with chronic hepatitis B

Objective： To study the clinical value of the correlation of combined detecting preS1 antigen and Hepatitis B core antigen in the diagnosis and treatment of the hepatitis B. Methods： It had been carried out the detection of HBV DNA for 568 samples of the chronic hepatitis B patients and 486 dermal controls by means of the Quantitative Real-Time TaqMan PCR （FQ-PCR）, HBV preS1 combined with HBcAg and hepatitis B serum markers individually were detected by HBV NRAg ELISA. HBVDNA, HBV-M, ALT, AST were also detected. Results： In 568 patients with chronic hepatitis B serum samples, the detection rates of HBV NRAg and HBeAg were separately 98.3% and 37.4%. There was significant correlation between the two（P〈0.05）. In 1 054 serum samples, the detection rates of ALT and AST were significantly increased in HBV NRAg positive group, the difference had statistical significance （P〈0.05）. Conclusion： HBV NRAg ELISA can be a useful supple mentary for clinical judgement of HBV infection and replication.
