细胞毒药物对急性白血病原位凋亡的诱导作用

摘    要:目的观察细胞毒化疗对急性白血病原位细胞凋亡的影响。方法用碱性磷酸酶-抗碱性磷酸酶(APPAAP)免疫酶标和DNA末端原位标记(ISEL)双重染色方法检测38例急性白血病诱导化疗前后骨髓涂片中细胞凋亡程度及细胞类型,初治组17例与21例经治病例对照。结果化疗后凋亡明显增加,与化疗前比较有极显著差异(P<0.001),凋亡指数(AI)>5%者平均原始细胞下降指数(MBDI)为28.6%,AI<5%者平均MBDI仅为4.9%。凋亡细胞大多数为白血病细胞。非白血病细胞凋亡亦有增加。结论细胞毒化疗的主要机理之一为促进细胞凋亡。化疗后全血细胞下降与正常造血细胞凋亡增加有关。

Induction of Cytotoxic Drugs to in situ Cell Apoptosis in Acute Leukemias
Abstract:Objective To verify the influence of cytotoxic chemotherapy to in situ apoptosis inacute leukemia(AL). Methods A double-stained way of APAAP and in situ DNA end labeling(ISEL) were used to detect apoptosis characteristics in 38 cases of acute leukemia, including 17 untreat-ed and 21 treated patients. Results The treated cases had a greatly increased mean apoptosis index(AI) ( P < 0. 001 ) which showed a positive correlation with marrow blast decrease index ( MBDI ).Apoptosis occurred mainly in the leukemical cells but a simultaneous growth apoptosis of non- leukemiacell was also observed. Conclusion One of the important mechanisms of cytotoxic chemotherapy is topromote leukemia cell apoptosis and chemotherapy-induced pancytopenia was caused partly by increasedapoptosis of non- leukemia cells.
