医院药学新的工作模式—药学监护

药学监护（Pharmaceutical Care)是医院药学工作的重要组成部分。临床药师在药学监护工作中的主要任务是：与患者或医、护人员共同设定并实施治疗计划。认真观察患者出现药物治疗效果的全过程。发现、解决并预防治疗用药方面出现的问题，并做好相应的记录，临床药师通过药历管理、服药指导和TDM（治疗药物监测）等方面的工作对患者进行药学监护，因为是直接接触患者，并且要明确、及时地解决有关问题，所以对从事这一工作的临床药师，也有更高的要求。

Development of Pharmaceutical Care

Interdisciplinary mutual promotion and development between new and high Sci - Tech methods and theoretical, practical, and research fields of pharmaceutical care are expected to importantly contribute to effectiveness and efficiency building as well as more and more safer application of therapeutically useful drugs, especially to increasing an understanding of many variables which influence the body response to drugs, such as: individual differences in the rates of removal of drugs from the body; the presence of alterations of hepatic or renal function, or of acid base balance; the presence of extremes of body weight and fat stores; the status of endocrine function, particularly thyroid; individual differences in the rates of intestinal absorption or of binding of drugs to plasma proteins; allergic reactions to drugs.
