75岁以上症状性脑动脉狭窄患者血管内支架治疗的临床观察

目的评价年龄≥75岁症状性脑动脉狭窄患者血管内支架治疗的安全性和疗效。方法选择症状性脑动脉狭窄患者112例,根据年龄分为≥75岁组33例,<75岁组79例,对比分析2组患者支架置入术后,支架置入成功率、围术期并发症及随访期间支架置入后再狭窄率、改良Rankin评分量表(mRS)和临床终点事件。结果≥75组和<75岁组支架治疗成功率分别为97.1%和97.5%,2组围手术期并发症发生率分别为15.2%和11.4%。术后随访期间,2组支架置入后动脉再狭窄率分别为12.1%和11.4%,≥75岁组生存率明显低于<75岁组(χ2=2.013,P=0.029)。结论年龄≥75岁症状性脑动脉狭窄患者生存率低于<75岁患者,但在手术成功率、并发症发生率、动脉再狭窄率及mRS评分无差异,支架置入治疗≥75岁症状性脑动脉狭窄是安全、有效的治疗策略。

Stent implantation in patients with symptomatic cerebral artery stenosis aged over 75 years

Objective To assess the safety and effect of stent implantation in patients with symptomatic cerebral artery stenosis aged over 75 years.Methods One hundred and twelve patients with symptomatic cerebral artery stenosis were divided into ≥75 years old group(n=33) and <75 years old group(n=79).Their success rate of stent imlantation,incidence of perioperative complications,and restenosis during follow-up period,modified Rankin scale(mRS) scores and events at clinical end point were compared and analyzed.Results The success rate of stent implantation was 97.1% and 97.5% respectively,the incidence of peroperative complications was 15.2% and 11.4% respectively,and the incidence of restenosis was 12.1% and 11.4% respectively during the follow-up period,in ≥75 years old group and <75 years old group(P<0.05).The survival rate was significantly lower in ≥75 years old group than in <75 years old group(χ2=2.013,P=0.029).Conclusion The survival rate is significantly lower in ≥75 years old group than in <75 years old group.However,no significant difference can be observed in success rate of stent implantation,incidence of peroperative complications and restenosis during the follow-up period,and the mRS scores between the two groups.Stent implantation is a safe and effective procedure for patients with symptomatic cerebral artery stenosis aged over 75 years.
