帕金森病运动并发症的调查分析

目的探讨帕金森病患者运动并发症的发生率及其影响因素。方法收集72例原发性帕金森病患者的病史资料,应用剂末现象-9项问卷、修订版不自主运动评定量表筛选剂末现象和异动症,进行统一帕金森病评定量表第3部分(UPDRSⅢ)、Hoehn-Yahr量表评估。结果 72例帕金森病患者中,运动症状波动发生率为26.4%,异动症发生率为12.5%,两者在接受左旋多巴治疗>5年的患者中发生率分别为43.3%、27.8%。与不伴运动并发症比较,伴发运动并发症病程、左旋多巴疗程、Hoehn-Yahr分级明显升高(P<0.05)。发病年龄、左旋多巴疗程及左旋多巴每日剂量为运动并发症的独立影响因素。结论运动并发症是帕金森病的常见表现,发病年龄越早,左旋多巴每日剂量越大、疗程越长,越容易出现运动并发症。

Motor complications in patients with Parkinson's disease

Objective To study the incidence of motor complications in patients with Parkinson's disease and its influence factors.Methods Medical histories of 72 primary Parkinson's disease patients were collected.Their wearing-off phenomina and dyskinesia were observed according to the wearing-off questionnaire with 9 items and modified abnormal involuntary movement scale and assessed following the unified Parkinson's disease rating scale 3 and Hoehn-Yahr scale.Results The incidence of motor complications and dyskinesia was 26.4% and 12.5% respectively in the 72 Parkinson's disease patients before treatment,and was 43.3% and 27.8% respectively 5 years after levodopa therapy.The course of disease and the course of levodopa treatment were significantly longer whereas the Hoehn-Yahr staging was significantly higher in patients with motor complications than in those without motor complications(P<0.05).Age at onset of disease,dosage of daily levodopa and course of levodopa treatment were the independent factors for motor complications.Conclusion Motor complications are common in Parkinson's disease patients.The earlier the disease occurs,the higher the daily levodopa dosage is used.The longer the course of disease is,the oftener the motor complications develop.
