二级医院介入社区高血压病长期多因素综合干预研究

目的探讨二级医院在社区高血压防治尤其是高危患者的二期预防方面的作用。方法在太仓市城厢地区把各社区高血压高危患者采取“心血管病人之友”俱乐部方式组织起来，进行了平均近5年的多因素综合干预。结果报告的145例患者血压、体重指数、甘油三酯有明显下降，患者接受干预的依从性高，80．7％的患者血压控制在140／90mmHg以下。31例合并糖尿病患者血糖明显下降，血糖控制率明显提高。每人的平均降压费用仅2．30元／d，6年来未有心血管死亡的发生，心血管事件显著减少。结论二级医院介入社区高血压人群的防治有明显的效果，是一种适合我国国情的心脑血管疾病防治模式。

District Hospital's Participation in Long Term Comprehensive Intervention of Hypertension

Objective To investigate the role of district hospital in prevention of hypertension, especially in high risk patients. Methods In Chengxiang town of Taicang city, we organized the hypertensives in a 'cardiovascular disease club' and gave them comprehensive intervention about 5 years. Results The blood pressures, BMIs, serum triglycerides of the 145 patients were decreased significantly. Blood pressures were lewer than 140/90 mm Hg in 80.7% of the patients. The serum glucoses of 31 patients complicated with diabetes decreased significantly. The average cost of antihypertensive drugs was 2.30 yuan per day. There was no cardiovascular death since 1998 in this cohort of patients. Conclusion The effect of district hospital's participation in control and secondary prevention of hypertension is evaluated. It may serve as an a appropriate model for prevention and treatment for cardiovascular disease in our country.
