急性梗阻化脓性胆管炎的内镜治疗

目的 探讨经内镜鼻胆管引流(ENBD)在治疗急性梗阻化脓性胆管炎(AOSC)中的作用和价值。方法 52例AOSC的病人，经逆行胰胆管造影(ERCP)后，病情危重直接行ENBD；病情允许的行十二指肠乳头括约肌切开(EST)、取石及ENBD同步进行。结果 31例直接行ENBD、21例EST、取石和ENBD同步进行；23例ENBD后择期EST及取石治疗、3例乳头肿瘤的病人病情缓解后转外科手术，无死亡。结论 ENBD是一种安全有效的胆道引流手段，临床上可以广泛地应用于AOSC。

The endoscopic therapy of acute obstructive suppurative cholangitis

Objective To evaluate the effects and value of ENBD on the therapy of AOSC. Methods After ERCP were performed on 52 patients with AOSC,ENBD were directively performed for severe patients,and EST, taking stone and ENBD were synchronously performed if situation was permitted. Results ENBD was directively performed on 31 cases EST,taking stone and ENBD were synchronously practised on 21 cases, EST and taking stone were practised in selective period after ENBD on 23 cases 3 cases of patients with papillary tumor were transmitted to surgery department for operation. There was no death.Conclusions ENBD is a safe and effective measure for biliary tract draining. It could be widely applied on clinic on patients with AOSC.
