黄精四草汤加味治疗缺血性脑血管病临床研究

目的:观察黄精四草汤加味治疗缺血性脑血管病的临床疗效及对患者血流动力学的影响。方法:将70例缺血性脑血管病患者按照信封法随机分为研究组和对照组,各35例。对照组采用常规西医治疗,研究组在对照组的基础上联合黄精四草汤加味治疗,疗程均为1个月。观察比较2组中医证候积分以及脑血流动力学患侧大脑中动脉(MCA)的峰流速(Vp)、平均流速(Vm)、峰流速对称性差值(DVp)、平均流速对称性差值(DVm)]改善程度。结果:研究组总有效率为88.57%,对照组为68.57%,2组比较,差异有统计学意义(P<0.05)。治疗后,研究组中医证候口舌歪斜、语言功能、半身不遂、偏身麻木、头晕耳鸣、手足心热、口干咽干等评分及舌脉评分均明显低于对照组,Vp、Vm明显高于对照组,DVp、DVm明显低于对照组,差异均有统计学意义(P<0.05)。结论:黄精四草汤加味治疗缺血性脑血管病患者其临床症状可得到有效控制,脑血流动力学指标显著恢复。

Clinical Study on Modified Huangjing Sicao Tang for Ischemic Cerebrovascular Disease

Objective:To observe the clinical effect of modified Huangjing Sicao tang for ischemic cerebrovascular disease and its effect on hemodynamics.Methods:A total of 70 cases of patients with ischemic cerebrovascular disease were randomly divided into the study group and the control group according to the envelop method,35 cases in each group.The control group was treated with routine western medicine,and the study group was additionally treated with modified Huangjing Sicao tang based on the treatment of the control group.Both groups were treated for one month.The improvement of Chinese medicine syndrome scores was observed and compared between the two groups.The improvement of indexes of cerebral vascular hemodynamics,including the peak velocity(Vp)of middle cerebral artery(MCA)on the affected side,mean velocity(Vm),symmetric difference of peak velocity(DVp),and symmetric difference of mean velocity(DVm)was observed and compared between the two groups.Results:The total effective rate was 88.57%in the observation group,and 68.57%in the control group,the difference being significant(P<0.05).After treatment,the Chinese medicine syndrome scores of deviated mouth and tongue,linguistic function,hemiplegia,hemianesthesia,dizziness and tinnitus,heat in the palms and soles,thirst and dry throat,and tongue manifestation in the study group were significantly lower than those in the control group;Vp and Vm were significantly higher than those in the control group;DVp and DVm were significantly lower;the difference was significant(P<0.05).Conclusion:Modified Huangjing Sicao tang can effectively control the clinical symptoms of patients with ischemic cerebrovascular disease,and help achieve a significant recovery of cerebral vascular hemodynamic indexes.
