双水平气道正压通气对慢性阻塞性肺疾病所致呼吸衰竭的临床研究

目的 :研究双水平气道正压通气对慢性阻塞性肺疾病所致呼吸衰竭的临床应用。方法 :应用BiPAP呼吸机对 2 5例慢性阻塞性肺疾病并发急性呼吸衰竭患者作经鼻罩行双水平气道正压通气 (试验组 ) ,并以同期病情相当的 2 3例患者采用常规治疗作对照 ,观察两组的疗效和血气分析情况。结果 :治疗前后血气分析、呼吸频率和心率的变化表明 :两组治疗后的PaO2明显上升 (P <0 .0 1) ,试验组上升更高。试验组PaCO2 比治疗前明显下降 (P <0 .0 5 ) ,对照组PaCO2 治疗前后差别不显著 (P>0 .0 5 )。试验组的呼吸频率、心率比治疗前明显减慢 (P <0 .0 5 ) ,对照组的呼吸、心率减慢差别不显著。试验组病情好转时间比对照组短。结论 :双水平气道正压通气对慢性阻塞性肺疾病所致呼吸衰竭有重要的应用价值。

Clinical Analysis of the Application of Bi - level Positive Airway Pressure in Pespiratory Failure Patients with Chronic Obstructive Lung disease

Objective: To study the clinical application of bi-level positive airway pressure ventilation on respiratory failure caused by chronic obstructive pulmonary diseases. Methods: 25 cases of chronic obstructive pulmonary diseases complicated with acute respiratory failure were treated with nasal mask bi-level positive airway pressure ventilation (treatment group). 23 cases with similar state of an illness were treated with conventional therapy (control group). The clinical effects and arterial blood gas were measured after treatments. Results: After treatments, both groups got a notable increase of PaO_2 (P<0.01), but the treatment group rose more than control group. PaCO_2, respiratory frequency and heart rate were all significantly decreased in treatment group (P<0.05) but not in control group (P>05). The improvement course was shorter in the treatment group than the control group. Conclusion:Bi-level positive airway pressure ventilation has an important application value on respiratory failure caused by chronic obstructive pulmonary diseases.
