疼痛教育对腹部手术患者疼痛控制的影响

摘    要:目的观察对患者开展疼痛知识必要的术前教育后,其术后疼痛的控制效果。方法对我院自2008年11月至2011年11月以来,于我科治疗的168例实施腹部手术的患者临床资料进行回顾性分析,将其依据住院病区分为A(术前教育88例)、B(非术前教育80例)两组,对A组患者予以疼痛知识的宣讲,并于术后对两组患者进行疼痛评估。结果 A组患者术后2h的VAS评分是(4.4±2.4)分,B组患者术后2h的VAS评分是(4.6±2.2)分,不具差异(P>0.05)。A组患者术后12h的MPQ总分是(17.2±12.3)分,B组患者术后12h的MPQ总分是(21.4±11.7)分,差异显著(P<0.05)。A组患者术后12h应用的镇痛药剂量是(25.5±9.1)mL,B组患者术后12h应用的镇痛药剂量是(36.4±10.5)mL,差异显著(P<0.05)。A组患者中,共22人次进行了镇痛剂的肌注,B组患者中,共46人次进行了额外镇痛剂的肌注,差异显著(P<0.05)。结论心理因素对患者疼痛反应至关重要,开展术前教育,能够提高患者的认知水平,并且缓解患者的紧张情绪,还能够帮助患者对术后的疼痛予以控制,对提高临床护理质量意义重大。

Impact of Pain Education on Pain Control in Patients Undergoing Abdominal Surgery
Abstract:Abstrac: Objective Observation of the patient to carry out the pain knowledge necessary preoperative education,postoperative pain control effect.Method Were retrospectively analyzed the clinical data of 168 patients undergoing abdominal surgery in our hospital from November 2008 to November 2011,at department treatment based on the hospital ward is divided into A(preoperative education 88 cases),B(80 cases) non-preoperative education groups,group A patients to be pain knowledge of preaching,and two groups of patients in the postoperative pain assessment.Result VAS scores in group A patients after surgery for 2h(4.4±2.4) of Group B patients the VAS score of 2h(4.6±2.2) points,not a difference(P>0.05).MPQ total score in group A patients 12h(17.2±12.3) points,the MPQ score in Group B patients 12h(21.4±11.7),the difference was significant(P<0.05).Group A patients after 12h application of analgesic dose(25.5±9.1) ml,group B patients after 12h application of analgesic dose(36.4±10.5) ml,a significant difference(P<0.05).Group A patients,a total of 22 cases of intramuscular injection of analgesics,Group B patients,a total of 46 people additional intramuscular injection of analgesics,the difference was significant(P<0.05).Conclusion Psychological factors in patients with pain response is critical to carry out preoperative education,to improve the patient’s cognitive level and ease the tension of the patients,can also help patients with postoperative pain be controlled to improve the quality of clinical care is of great significance.
