老年社区获得性肺炎患者中血清急性时相蛋白的临床动态分析

目的:探讨血清急性时相蛋白对老年社区获得性肺炎的临床应用价值。方法:对80例老年社区获得性肺炎患者作为肺炎组进行回顾性分析,以同期在我院健康门诊体检的50例老年人作为对照组,分别记录肺炎组患者治疗前、治疗第4天、治疗第10天的C反应蛋白(CRP)、前白蛋白(PA)、白蛋白(ALB)、白细胞计数(WBC)、中性粒细胞百分比(N)、痰培养的情况,并进行统计学分析。结果:肺炎组患者CRP浓度(7.42±3.74mg/dL]显著高于对照组(0.21±0.15)mg/dL](P<0.01),有效治疗后显著降低(P<0.01),且CRP的阳性率明显高于WBC、N和痰培养;另外CRP和PA、ALB呈反向变化。结论:CRP可作为诊断肺部感染和观察治疗效果的敏感指标,PA是一种负急性时相反应蛋白,和CRP一起可作为判断炎症程度的可靠指标。

The dynamic analysis of clinical on serum acute phase protein of the Prognosis of Aged patients with Community Acquired Pneumonia

ABSTRACT] Objective: To evaluate the clinical values of acute phase protein in the diagnosis of Aged patients with Community Acquired Pneumonia. Methods:80 Aged patients with Community Acquired Pneumonia were as group therapy and 50 healthy elder persons were as the control group .Their serum values of CRP、PA、ALB、WBC、N、sputum culture and positive rates of CRP were analyzed. Results :The levels of CRP (7.42 3.74 mg/dl) in the therapy group were much higher than that in the control group (0.21 0.15 mg/dl),(P<0.01). Positive rates of CRP were much higher than that of WBC、N and sputum culture. Besides, CRP and PA、ALB were reverse changes. Conclusion: Dynamic analysis of CRP and PA provides an important means in evaluating the prognosis of CAP in aged patients, especially when symptoms were atypical. It is recommended to use them jointly.
