摘    要:目的探讨早期骨性Ⅲ类错患者上腭快速扩弓联合前方牵引矫治后咽腔的改变。方法选择2007年6月至2009年5月大连医科大学附属第一医院口腔正畸门诊收治的早期骨性Ⅲ类错患者30例,行上腭快速扩弓联合前方牵引治疗,并测量矫治前后各指标变化。结果与矫治前比较,矫治后上齿槽座点到Y轴的距离(Y-A)增加,下齿槽座点到Y轴的距离(Y-B)和颏前点到Y轴的距离(Y-Pog)减小,上咽腔宽(UPW)、中咽腔宽(MPW)增加,鼻咽部面积(nasopharynx)和口咽上部面积(oropharynx1)增加(均P<0.01);下咽腔宽(LPW)、口咽下部面积(oropharynx2)及头位角(SN-CVT)变化差异无统计学意义(均P>0.05)。结论上颌前方牵引能改变咽腔大小。

Abstract:Objective To evaluate the outcom of treatment with rapid maxillary expansion(RME)and maxillary protraction in patients with a skeletal class Ⅲ malocclusion diagnosed as maxillary deficient on upper airway dimensions.Methods Thirty patients with skeletal Class Ⅲ malocclusions admitted to department of Orthodontics,the First Affiliated Hospital of Dalian Medical University,from June 2007 to May 2009 were selected,and treated using RME and face mask.Lateral cephalometric radiographs of 30 patients were examined.Results The maxillary protraction appliances caused the maxilla to move forward and the mandible to rotate in a clockwise direction.As a consequence,Y-A distance(A-point to Y-axis)increased,whereas Y-B(B-point to Y-axis)and Y-Pog distances(Pogonion to to Y-axis)decreased.The airway passage showed upper and middle pharyngeal width increase at the significant level(P 0.01).Significant increases were also obtained in the measurements of the area of the nasopharynx and oropharynx 1.Insignificant increases were present in the lower pharyngeal width,oropharynx 2 areas and SN-CVT angle.Conclusion The size of the upper airway can be increased by means of MPA application.
